摘 要:目的探讨小儿胸部数字化X线摄影(digital radiography,DR)数据库的建立与质量控制体系的应用。方法抽取2017年5月—2018年5月DR数据库与质量控制体系建立后小儿胸部DR影像资料100份为研究组,抽取2016年4月—2017年4月DR数据库与质量控制体系建立前小儿胸部DR影像资料100份为对照组。统计小儿DR数据,并建立数据库,同时分析拍摄小儿DR过程中,体位、照射野、伪影异物及呼吸运动等对成片质量的影响,并逐步改善影像后处理技术,分析影像质量不佳原因,探究质量控制体系建立的临床意义。结果研究组DR胸片质量评分明显高于对照组,研究组甲级片所占比例明显高于对照组,而丙级片所占比例明显低于对照组(P<0.05)。降低图像质量的因素主要包括体位摆放不正、移动伪影、呼吸运动模糊、影像后处理不恰当、呼气曝光、照射野偏大。研究组体位不正、移动伪影、呼吸运动模糊、呼气曝光及照射野偏大所占比例明显低于对照组(P<0.01)。研究组误诊率明显低于对照组(P<0.05)。结论小儿胸部DR数据库与质量控制体系建立后,拍摄质量明显提升,故小儿胸部DR数据库与质量控制体系值得推广应用。Objective To explore the establishment of digital radiography(DR)database in children and the application of quality control system.Methods From May 2017 to May 2018,100 children′s chest DR image data after the establishment of DR database and quality control system were taken as the study group,and from April 2016 to April 2017,100 children′s chest DR image data before the establishment of DR database and quality control system were taken as the control group.We have made statistics of DR data of children,and established a database.At the same time,we have analyzed the influence of body position,irradiation field,artifact foreign body and respiratory movement on the quality of children′s DR.We have gradually improved image post-processing technology,analyzed the reasons for poor image quality,and explored the clinical significance of establishing quality control system.Results The proportion of grade A tablets in the study group was significantly higher than that of the control group,while the proportion of grade C tablets was significantly lower than that of the control group(P<0.05).The main factors that reduce the image quality included the patient′s improper position,moving artifact,respiratory motion blur,image post-processing,expiratory exposure and large irradiation field.The proportion of the patient′s improper position,moving artifact,respiratory motion blur,image post-processing,expiratory exposure and large irradiation field in the study group were lower than that in the control group(P<0.0.05).The misdiagnosis rate of the study group was significantly lower than that of the control group(P<0.05).Conclusion After the establishment of the children′s chest DR database and quality control system,the quality of shooting is obviously increased.The children′s chest DR database and quality control system should be promoted and applied.
