高原地区先天性动脉导管未闭介入治疗方法与疗效分析

摘　　要：目的探讨高原地区先天性动脉导管未闭介入治疗方法与疗效。方法通过回顾分析我院87例患者资料,术前经血常规、肝肾功能、凝血功能、甲状腺功能、胸片、超声心动图检查确诊为动脉导管未闭,无手术禁忌症,术中术后监测肺动脉压及主动脉造影,术后随访1~12个月。结果87例动脉导管未闭均封堵成功。监测肺动脉压力变化,均明显下降,肺动脉收缩压降低幅度为原来压力的20%或下降30 mmHg,释放封堵器后,封堵器形态良好,无封堵器移位;左心室明显缩小,恢复到正常范围,无并发症发生。结论介入治疗对高原先天性动脉导管未闭具有成功率高,风险小,痛苦小,无需开刀、住院时间短、恢复快等优点,无疑是高原先心病治疗的首选。Objectives This article was conducted to investigate the application of self-expandable occlude for transcatheter closure of patent ductus arteriosus,as well as the methods and results.Methods By retrospective analysis the information of 87 patients in our hospital.Based on blood routine,liver renal function,coagulation function,thyroid function,chest X-ray and echocardiography,that used for diagnosed as patent ductus arteriosus.No operation contraindication.Monitored pulmonary artery pressure and aortography during and after closure,and follow-up for 1~12 months.Results 87 cases of patients were successful occlusion.Pulmonary artery pressure in all patients were showed decrease,in some patients with severe pulmonary hypertension were showed down 30mmHg or 20 percent.Meanwhile,after released the occluder,no occluder shift,and left ventricular narrowed considerably back to their normal range without complication.Conclusion Self-expandable occlude for transcatheter closure of patent ductus arteriosus had a high success rate,litter risk,without shorten hospitalized period,recovered soon and so on.It is undoubtedly the preferred choice for congenital heart disease in plateau section.
