摘 要:目的研究羟丁基几丁糖膜修复兔硬脑膜缺损的效果。方法取健康成年新西兰兔15只,颅中线两侧对称造成0.8 cm×0.8 cm硬脑膜缺损,左侧(实验组)使用羟丁基几丁糖膜覆盖硬脑膜缺损,右侧(对照组)使用生物膜(牛肌腱I型胶原)覆盖缺损,术后30、90、180 d分别处死5只实验动物,观察手术区域的大体情况及切取手术区域组织行HE染色,行成纤维细胞计数。结果实验组术后30、90、180 d,均可见羟丁基几丁糖膜下半透明新生膜形成,与周围组织无粘连;术后180 d,新生膜中成纤维细胞计数与正常兔硬脑膜比较差异无统计学意义(P>0.05),大体观察及HE染色下结构与正常兔硬脑膜相似性高。对照组术后90 d生物膜被吸收,手术区域形成一较厚不透明结缔组织覆盖脑表面;术后180 d,新生结缔组织成纤维细胞计数与正常兔硬脑膜比较差异有统计学意义(P<0.05),大体观察及HE染色观察较正常兔硬脑膜相似性低。结论羟丁基几丁糖膜修复兔硬脑膜缺损,膜下有新生膜形成,新生膜与兔正常硬脑膜相似性高,可以达到修复硬脑膜缺损的要求。Objective To study the effect of hydroxyl butyl chitosan membrane on repairing dural defects in rabbits.Methods Fifteen healthy adult New Zealand rabbits were selected to prepare the bilateral dural defect(0.8 cm×0.8 cm in size)via midline incision of head.Defect of the left was repaired with hydroxyl butyl chitosan membrane as the experimental group,defect of the right was repaired with biological membrane as the control group.At 30,90 and 180 days after operation,5 rabbits were sacrificed for sampling to observe the general situation and HE staining of the operation area,and count the fibroblasts.Results At 30,90,and 180 days after surgery in the experimental group,there was a new membrane that showed no adhesion with the surrounding tissue formed under the hydroxyl butyl chitosan membrane.180 days after the operation,there was no significant statistical difference with the new membrane and normal rabbit dura in the number of fibroblasts.The appearance of the new membrane and the structure of HE staining were similar to that of normal rabbit dura mater.In the control group,a thick opaque connective tissue was formed to cover the surface of the brain and the biological membrane was absorbed 90 days after the operation.180 days after the operation,there was significant statistical difference with the connective tissue and normal rabbit dura in the number of fibroblasts.The appearance of the connective tissue and the structure of HE staining were obviously different with that of normal rabbit dura mater.Conclusion There is a new membrane formed under hydroxyl butyl chitosan membrane.The new membrane is similar to the normal rabbit dura mater.The dural defects in rabbits can be achieved by the use of hydroxyl butyl chitosan membrane.
