改良的龟头减容结合舌黏膜替代尿道腔治疗尿道外口狭窄

摘　　要：目的介绍一种改良的龟头减容技术,即经尿道外口行舌黏膜移植治疗尿道外口及舟状窝狭窄的新方法。方法回顾性分析2018年1月至2019年1月于吉林大学中日联谊医院泌尿外科行经尿道外口舌黏膜移植治疗尿道外口及舟状窝狭窄的14例患者的围手术期相关资料,记录术中情况、术后排尿情况以及并发症,术后随访6个月至1年。结果 14例患者手术全部成功,术后3周拔管恢复正常排尿,拔管后2周测最大尿流率17.6~28.3mL/s,彩超测残余尿均在50mL以下。尿道外口外观接近正常裂隙状开口且其他部位无手术切口,患者满意。结论改良的龟头减容结合舌黏膜移植治疗尿道外口及舟状窝狭窄,操作简单,其他部位无手术切口,患者对尿道外口外观满意,在把握好手术适应证的前提下可作为尿道外口及舟状窝狭窄的一种手术方式。Objective To introduce a new method of treating external orifice stricture with tongue mucosa transplantation.Methods The clinical data of 14 patients with external orifice and navicula stricture treated with tongue mucosa transplantation during Jan.2018 and Jan.2019 were retrospectively analyzed.The perioperative data,intraopertaive data,postoperative urination and complications were recorded.The follow-up ranged from 6 months to 1 year.Results All operations were successful.All patients regained covered normal urination after removal of the catheter three weeks after operation.The maximum urinary flow rate was 17.6-28.3 mL/s and the residual urine volume was less than 50 mL two weeks after extubation.The appearance of the urethral orifice looked normal,and there was no surgical incision in other parts.All patients were satisfied.Conclusion It is safe and effective to treat external orifice and navicula stricture with glans volume reduction and tongue mucosa transplantation.There is no surgical incision in other parts,and the appearance of the external orifice is satisfactory.This treatment method can be a new choice when the indications are well selected.
