渝东北地区1 503例泌尿系结石患者的结石成分分析

摘　　要：目的探讨渝东北地区泌尿系结石成分构成现状,并为三峡库区居民结石预防和治疗提供理论依据。方法回顾性分析重庆三峡中心医院2016年4月至2018年12月收治的1 503例泌尿系结石患者的病历资料,获取患者性别、年龄、影像学、结石成分分析等临床资料,分析男女患者的结石成分特点及各类结石的分布情况。结果发病部位上下尿路结石比例11.12∶1。单一成分结石83例,混合成分结石1 420例。结石成分多样,主要结石成分为一水草酸钙(90.4%)、二水草酸钙(80.7%)和碳酸磷灰石(50.0%);男性患者二水草酸钙检出率高于女性(P<0.01),女性患者碳酸磷灰石及六水磷酸铵镁检出率高于男性(P<0.01,P=0.01),其余结石成分男女患者检出率无统计学差异。结论渝东北地区结石成分以草酸钙、碳酸磷灰石最常见,以混合结石为主,组成成分复杂。Objective To investigate the composition of urinary calculi in northeast Chongqing in order to guide the prevention and treatment.Methods The clinical data of 1 503 patients with urinary calculi admitted to our hospital during Apr.2016 and Dec.2018 were retrospectively analyzed,including gender,age,images and stone components,to analyze the characteristics of stone components and distribution.Results There were 1 379 cases of upper and 124 cases of lower urinary tract calculi(11.12∶1),including 83 cases of single-component calculi and 1 420 cases of mixed-component calculi.The detection rate of calcium oxalate monohydrate,calcium oxalate dehydrate and apatite carbonate was 90.4%,80.7% and 50.0%,respectively.The detection rate of calcium oxalate dihydrate was higher in male patients than in female patients(P<0.01),while the detection rates of apatite carbonate and magnesium ammonium phosphate hexahydrate were higher in female patients than in male patients(P<0.01,P=0.01),and there were no statistical differences in the detection rates of other components between male and female patients.Conclusion Calcium oxalate and apatite carbonate are the most common components of calculi in northeast Chongqing.Most calculi are mixed and complex.
