重症监护室神经疾病患者继发中枢神经系统感染的危险因素研究

摘　　要：目的分析重症监护室(ICU)神经疾病患者继发中枢神经系统感染(CNSI)的危险因素,以便为临床控制感染提供依据。方法选择2017年2月-2019年2月郑州大学第二附属医院ICU收治的神经疾病患者80例进行研究,纳入对象有完整资料,根据是否继发CNSI分为观察组(继发CNSI,n=10例)与对照组(未继发CNSI,n=70例)。对两组患者相关因素进行比较分析,同时对危险因素进行多因素回归分析。结果两组患者在年龄、急诊入院、合并高血糖、手术时间、放置引流管、引流管放置部位、引流管放置时间、是否行幕下开颅手术上比较差异显著(P<0.05);多因素Logistic回归分析显示年龄<60岁、放置引流管、幕下开颅手术是ICU神经疾病患者继发CNSI的危险因素(P<0.05)。结论ICU神经疾病患者继发NISI的危险因素较多,特别是年龄<60岁、放置引流管、是否行幕下开颅手术等情况下应重点注意。Objective Analyzing the risk factors of secondary central nervous system infection in patients with neurological diseases in intensive care unit to provide evidence for clinical infection control.Methods 80 patients with neurological diseases admitted to the ICU of the second affiliated hospital of zhengzhou university from February 2017 to February 2019 were selected for the study,and the included subjects had complete data.According to whether CNSI was secondary,the patients were divided into observation group(secondary CNSI,n=10 cases)and control group(non-secondary CNSI,n=70 cases).The relative factors between the two groups were compared and analyzed,and the risk factors were analyzed by multivariate regression analysis.Results There were significant differences between the two groups in age,emergency admission,hyperglycemia,operation time,placement of drainage tube,placement site of drainage tube,placement time of drainage tube,and subatentorial craniotomy(P<0.05).Multivariate Logistic regression analysis showed that before the age of 60,placement of drainage tube and subatentric craniotomy were risk factors for secondary CNSI in ICU patients with neurological diseases(P<0.05).Conclusion There are many risk factors for secondary NISI in ICU patients with neurological diseases,especially should pay more attention to the situation that under the age of younger than 60 years old,placement of drainage tube,subdententric craniotomy,etc.
