摘　　要：目的探讨促甲状腺素受体刺激性抗体(thyroid stimulating antibody,TSAb)[又称甲状腺刺激性免疫球蛋白(thyroid stimulating immunoglobulins,TSI)]检测在甲状腺疾病诊断中的临床价值。方法回顾性分析2019年6月至2019年7月期间1367例甲状腺疾病患者和3319例正常对照者的血清TSI水平,计算并比较甲状腺疾病各组与正常对照组的TSI水平及阳性率。结果临床甲状腺功能亢进组的TSI水平及阳性率显著高于其他甲状腺疾病组和正常对照组(均为P<0.01);其他各疾病组TSI水平均显著高于正常对照组(P<0.01);亚临床甲亢组、临床甲减组和亚临床甲减组的阳性率亦高于正常对照组(均为P<0.01)。结论血清TSI水平检测对甲状腺功能亢进诊断意义最为显著,在甲状腺疾病的临床诊断及鉴别诊断中具有重要意义。Objective We aimed to study the clinical value of testing thyroid stimulating antibody[TSAb,also known as thyroid stimulating immunoglobulins(TSI)]in the diagnosis of thyroid diseases.Methods TSI test results of 1367 patients with thyroid diseases and 3319 normal control subjects from June,2019 to July,2019 were respectively analyzed.TSI levels and positive rates were calculated and compared between groups.Results TSI levels and positive rates in the group with clinical hyperthyroidism were significantly higher than that of groups with other thyroid diseases and the normal control group(all P<0.01).TSI concentrations in other disease groups were significantly higher than that in the normal control group(P<0.01).TSI positive rates in groups with subclinical hyperthyroidism,clinical hypothyroidism or subclinical hypothyroidism were all significantly differed from that of the control group(all P<0.01).Conclusion Among thyroid diseases,testing TSI level is the most important for diagnosing hyperthyroidism.It holds promising clinical value for the diagnosis and differential diagnosis of thyroid diseases.
