摘 要:目的探究SLE患者外周血25(OH)D水平对免疫细胞Th/Ts平衡及TNF-α的影响。方法选取2017年2月至2019年4月我院收治的70例系统性红斑狼疮(SLE)患者作为SLE组,按照病情活动度分为缓解组(SLE缓解期,35例)和活动组(SLE活动期,35例)2个亚组,另选40例体检健康者作为对照组,检测各组外周血25(OH)D、炎性因子(TNF-α)及T细胞亚群水平。结果SLE组外周血25(OH)D水平、CD3^+、CD4^+、CD4^+/CD8^+(Th/Ts)比值均低于对照组,且缓解组高于活动组,差异具有统计学意义(P<0.05),各组CD8^+细胞计数比较差异无统计学意义(P>0.05);SLE组外周血TNF-α水平均高于对照组,且活动组高于缓解组,差异具有统计学意义(P<0.05);SLE患者外周血25(OH)D水平、CD3^+、CD4^+及CD4^+/CD8^+比值与SLEDAI评分呈负相关(r=-0.128,-0.080,-0.078,-0.207,P<0.05);SLE患者外周血TNF-α与SLEDAI评分呈正相关(r=0.279,P<0.05);SLE患者血清中25(OH)D水平与CD3^+、CD4^+及CD4^+/CD8^+呈正相关(r=0.257,0.438,0.301,P<0.05),而与TNF-α呈负相关(r=-0.362,P<0.05)。结论SLE患者外周血25(OH)D表达与T淋巴细胞亚群CD3^+、CD4^+、CD4^+/CD8^+呈正相关,与TNF-α呈负相关,推测25(OH)D可能参与调节机体T淋巴细胞亚群的平衡及TNF-α的表达从而影响SLE进程。Objective To investigate the effect of peripheral blood 25(OH)D level on Th/Ts balance and TNF-αin immune cells of SLE patients.Methods Seventy patients with systemic lupus erythematosus(SLE)admitted to our hospital from February,2017 to April,2019 were enrolled as the SLE group.According to the disease activity,they were divided into the remission group(SLE remission period,35 cases)and the activity group(SLE).During the active period,35 cases were divided into 2 subgroups,and 40 healthy subjects were selected as the control group.The levels of 25(OH)D,inflammatory factors(TNF-α)and T cell subsets in peripheral blood of each group were detected.Results The ratios of 25(OH)D,CD3^+,CD4^+and CD4^+/CD8^+(Th/Ts)in the peripheral blood of the SLE group were lower than those in the control group,and the remission group was higher than the active group.The differences were statistically significant(all P<0.05).There was no significant difference in CD8^+cell count between the two groups(P>0.05).The peripheral blood TNF-water level in the SLE group was higher than that in the control group,and the activity group was higher than the remission group(all P<0.05);SLE Peripheral blood 25(OH)D levels,CD3^+,CD4^+and CD4^+/CD8^+ratios were negatively correlated with SLEDAI scores(r=-0.128,-0.080,-0.078,-0.207,P<0.05),while peripheral blood TNF-in patients with SLE had a positive correlation with SLEDAI score(r=0.279,P<0.05).Serum 25(OH)D levels in SLE patients were positively correlated with CD3^+,CD4^+and CD4^+/CD8^+(r=0.257,0.438,0.301,P<0.05),but was negatively correlated with TNF-α(r=-0.362,P<0.05).Conclusion The expression of 25(OH)D in peripheral blood of patients with SLE is positively correlated with T lymphocyte subsets CD3^+,CD4^+,CD4^+/CD8^+,and negatively correlated with TNF-α.It is suggested that 25(OH)D could be involved in the regulation of T lymphocyte subsets balance and TNF-αexpression,which can affect the SLE process.
