成人髋臼骨折类型与闭孔内肌肿胀关系的影像学分析

摘　　要：目的探讨成人髋臼骨折类型与闭孔内肌厚度改变的关系。方法收集2008年10月至2018年9月间137例髋臼骨折患者的X线、CT及部分磁共振成像(MRI)图像资料进行回顾性分析,并对同时期207例成人的双侧闭孔内肌厚度在CT图像上进行测量、比较分析。结果正常双侧闭孔内肌厚度在CT图像上基本对称,髋臼骨折导致闭孔内肌肿胀增厚125例,其中复杂型骨折64例闭孔内肌均肿胀,简单型骨折73例中闭孔内肌肿胀者61例;闭孔内肌厚度无改变的12例(8.8%),均为简单型髋臼前壁骨折。X线片显示一侧闭孔内肌增厚,未显示骨折和错位征象,CT证实骨折存在者3例。结论闭孔内肌增厚是髋臼骨折的重要间接征象,闭孔内肌增厚与髋臼骨折的部位和类型有明显相关性,髋臼前壁骨折很少引起闭孔内肌增厚。Objective To study the relationship between type of the acetabular fracture and the changes of muscle thickness of musculus obturator internus in adults. Methods Imaging of 137 patients with acetabular fracture from October 2008 to September 2018,including X-ray, CT, MRI image data,was retrospectively collected and analyzed. And the thickness of the bilateral musculus obturator internus in 207 normal adults were also measured as comparison on CT images. Results The thickness of the normal musculus obturator internus is symmetrical in the CT image. Acetabulum fracture caused swelling and thickening of musculus obturator internus in 125 cases, including 64 cases with complex type fractures and 61 cases with simple type fractures. 12 cases with no change in thick-ness of musculus obturator internus were all simple type fractures of acetabular anterior wall.The X-ray showed a thickening of the imusculus obturator internus in one side without signs of fracture and dislocation in 3 cases, howev-er, CT confirmed the existence of fracture. Conclusion The thickening of the musculus obturator internus is an important indirect sign of acetabular fracture. The thickening of the musculus obturator internus muscle is obviously re-lated to the position and type of acetabular fracture. Acetabular anterior lip fractures are rarely associated with thick-ening of the musculus obturator internus.
