滤过泡针分离术治疗小梁切除术后白内障超声乳化术致眼压失控患者的临床观察

摘　　要：目的探讨滤泡针分离术治疗小梁切除术后白内障超声乳化术致眼压失控患者的临床效果。方法回顾性分析2016年1月至2018年12月于江苏省人民医院眼科就诊的小梁切除术后白内障超声乳化术致眼压失控患者,共17例(17只眼),均采用滤过泡针分离术治疗,治疗后1 d、7 d、 1个月、 3个月、 6个月随访,观察视力、眼压和并发症等情况。结果白内障术前、滤过泡分离术治疗前、治疗后1 d、1周、1个月、3个月和6个月眼压分别为(13.5±2.1)、(40.4±6.5)、(15.5±2.5)、(16.1±2.3)、(16.2±2.1)、(15.6±2.4)、(15.8±2.1) mmHg,治疗后眼压和治疗前比较均明显下降,统计学比较有显著性差异(P<0.01),滤过泡分离后滤过泡均能重新形成,视力较治疗前明显进步。结论滤过泡针分离术治疗小梁切除术后白内障超声乳化术致眼压失控疗效显著,值得推广。Objective To investigate the treatment effect of filtered bleb separation on acute intraocular hypertension after phacoemulsification among patients with previous trabeculectomy.Methods We retrospectively analyzed 17 patients(17 eyes) with a history of trabeculectomy who developed acute elevated intraocular pressure(IOP) due to phacoemulsification from January 2016 to December 2018 in Jiangsu Province Hospital. All patients were treated with filtered bleb separation. Visual acuity, IOP and complication were recorded before and at 1 day, 1 week, 1 month, 3 months, and 6 months after the treatment. Results The IOP before phacoemulsification, before filtered bleb separation, at 1 day, 1 week, 1 month, 3 months and 6 months after the filtered bleb separation was 13.5±2.1, 40.4±6.5, 15.5±2.5, 16.1±2.3, 16.2±2.1, 15.6±2.4 and 15.8±2.1 mmHg, respectively. After the treatment, IOP decreased significantly(P<0.01), and visual acuity improved. The morphology of filtered bleb can be restored for all patients after the treatment. Conclusions Filtered bleb separation is effective in treating acute intraocular hypertension after phacoemulsification among patients with previous trabeculectomy.
