Pentacam测量角膜屈光手术后角膜屈光力准确性的研究

摘　　要：目的探讨Pentacam眼前节分析系统测量角膜屈光手术后角膜屈光力的准确性。方法 61例(121只眼)角膜屈光手术(近视手术)后3个月,行Pentacam检查,取模拟角膜曲率计值(Sim K)、4.5等效角膜曲率计读数(EKR)、4.0区域真实净屈光力(TNP)、5.0 TNP值并与临床资料(CHM)法、Shammas法及Haigis-L法获得的K值相比较,方差分析,P<0.05为有统计学意义。结果术后3个月Pentacam测量的Sim K、4.5 EKR、4.0 TNP、5.0 TNP值以及CHM法、Shammas法和Haigis-L法获得的K值平均值和标准差分别为38.67±1.59、37.71±1.68、36.81±1.78、36.91±1.60、37.28±1.81、37.69±2.0和37.14±1.86;统计学处理,4.5 EKR与临床常用方法Shammas法和CHM法值比较,差异无统计学意义。结论 Pentacam测量的4.5 EKR值与临床常用的修正方法Shammas法和CHM法的K值差异没有统计学意义,理论上可以将4.5 EKR值作为角膜屈光手术后的角膜K值应用于现有的IOL度数计算公式,但在实际手术患者中应用方面,有待进一步研究。Objective To evaluate the accuracy of using Pentacam to measure the corneal refraction power after corneal refractive surgery.Methods A total of 121 eyes(61 patients) were measured by Pentacam at 3 months after the corneal refractive surgery(myopic surgery).The Sim K,4.5 EKR(equivalent keratometry reading, EKR),4.0 TNP(true net power,TNP) and 5.0 TNP measured by Pentacam were compared with the K values acquired by clinical history method(CHM),Shammas method and Haigis-L method. The analysis of variance was performed and P<0.05 was considered statistically significant.Results Three-months after the surgery,the mean values and standard deviations(SD) of the Sim K,4.5 EKR,4.0 TNP and 5.0 TNP measured by Pentacam and the K values acquired by CHM,Shammas method and Haigis-L method were 38.67±1.59,37.71±1.68,36.81±1.78,36.91±1.60,37.28±1.81,37.69±2.0 and 37.14±1.86,respectively.There was no statistically significant differences between the 4.5 EKR values and the results of Shammas method and CHM.Conclusions The value of 4.5 EKR measured by Pentacam was not significantly different from the K values measured by the Shammas method and CHM,which were commonly used in clinical practice.The value of 4.5 EKR could be applied to the current intraocular lens(IOL) power calculation formulas as the corneal K value after corneal refractive surgery theoretically,while further investigation is needed before application in surgical patients.
