摘 要:目的分析胎膜早破(PROM)与解脲支原体感染(UU)及羊膜基质金属蛋白酶9(MMP-9)表达的关联性。方法随机选取2017年1月~2018年12月我院收治的胎膜早破患者168例作为研究对象,即实验组,另选168例正常产妇作为对照组,对比两组产妇解脲支原体感染情况、分析MMP-9表达情况,并统计两组产妇妊娠情况。结果实验组解脲支原体感染率以及MMP-9表达积分分别为41.07%、4.89±0.34,显著高于对照组的6.54%、2.68±0.39,差异有统计学意义(P <0.05);实验组产妇产后出血、产程延长等不良妊娠结局显著高于对照组,差异有统计学意义(P <0.05)。而新生儿方面,仅新生儿死亡率,实验组与对照组无统计学差异(P> 0.05),其余新生儿并发症实验组的发生率显著高于对照组,差异有统计学意义(P <0.05)。结论胎膜早破与解脲支原体感染以及羊膜MMP-9表达具有相关性,解脲支原体感染引起MMP-9表达增高是胎膜早破的主要原因,临床上应该引起患者以及医护人员的注意。Objective To analyze the correlation between premature rupture of membranes(PROM) and ureaplasma urealyticum(UU) infection and MMP-9 expression(matrix metalloproteinase-9) of amniotic membrane. Methods 168 parturient women were selected as the control group. The infection status of ureaplasma urealyticum in the two groups was compared, the expression of MMP-9 was analyzed and the pregnancy status parturient women in two groups was counted. Results The infection rate of ureaplasma urealyticum and MMP-9 expression score in the experimental group were 41.07% and 4.89±0.34, respectively, which were significantly higher than those of the control group(6.54%, 2.68±0.39). The difference was statistically significant(P < 0.05). In the experimental group, maternal postpartum hemorrhage, prolonged labor and other adverse pregnancy outcomes were significantly higher than the control group, with statistically significant differences(P < 0.05). In terms of newborns, only neonatal mortality was found, with no difference between the experimental group and the control group(P > 0.05), while the incidence of other neonatal complications in the experimental group was significantly higher than that in the control group, with statistically significant differences(P < 0.05). Conclusion Premature rupture of membranes is associated with ureaplasma urealyticum infection and MMP-9 expression of amniotic membrane increased caused by ureaplasma urealyticum infection is the main cause of premature rupture of membranes. It should be paid attention to by patients and medical staff in clinical practice.
