孕妇及婴幼儿多种体液样本中巨细胞病毒感染情况的回顾性分析

摘  要:目的对孕妇乳汁、羊水及婴幼儿痰液、尿液和血液中的巨细胞病毒(HCMV)的DNA载量检测结果进行回顾性分析,以改进HCMV的临床检测工作。方法收集2017年1月~2018年6月在深圳市宝安区妇幼保健院经荧光定量PCR法进行HCMV DNA检测的样本4524例,按照人群特征、样本类型、感染症状及治疗监测等维度进行回顾性分析。结果孕妇的总体HCMV DNA检出率为7.39%,且无年龄差异;新生儿及1岁以内婴幼儿检出率44.78%,且无性别差异。不同体液的HCMV DNA检出率不同,乳汁的检出率最高为77.36%,痰液和尿液在30%以上,血清为12.06%,而羊水基本阴性。同一病例中,痰液和尿液的病毒载量高,而血清最低。有无临床症状病例的HCMV DNA检出率并无统计学差异。随出生时间的延长,巨细胞病毒的感染率明显增加,从先天感染期间的13.5%增加到生后感染期间的49.0%。通过血清监测HCMV的疗效发现HCMV DNA的转阴时间最短不到半月且易反复。结论多体液样本的同步检测、合理化选择和组合对于HCMV的检测和防治有重要的意义。Objective To evaluate the HCMV DNA detection in sputum, urine, blood of infants and the breast milk, amniotic fluid of their mothers with the aim of improving the clinical detection of HCMV. Methods A total of 4524 suspected HCMV infected cases detected by fluorescence quantitative PCR between January 2017 and June 2018 were retrospectively analyzed according to their demographics, sample types, symptom and therapy monitoring and other characteristics. Results The general positive rate of HCMV DNA of pregnant women was 7.39% without age difference.While that of neonates and infants within one year of age was 44.78%,which was identical in different sex.The positive rate differed in diverse body fluids,in fact,with 77.36% in breast milk as the highest,over 30% in sputum and urine,12.06% in serum while bare in amniotic fluid.Among different samples in the same patients,the DNA copy number of HCMV in sputum and urine were higher,while that in serum were relatively low.The positive rates were identical between asymptomatic and symptomatic cases.The infected rate of HCMV increased with the growth of infants significantly,which increased from 13.5% within congenital infection period to 49.0% in postnatal period.Positive case could turn to negative as soon as in half a month by monitoring the HCMV DNA copy number in serum. Conclusion The simultaneous,reasonable,combined choose of diverse samples were significant and important in the detection,preventionand cure of HCMV infection.
