摘　　要：目的分析急性创伤性硬膜下血肿治疗价值。方法选取2012年1月至2018年12月在海门市人民医院接受治疗的急性创伤性硬膜下血肿患者60例作为研究对象,将其中出现单侧瞳孔扩大的30例患者设为对照组,将未出现瞳孔扩大的30例患者设为观察组,两组患者均接受开颅血肿清除并行标准大骨瓣减压术治疗。比较两组患者的并发症发生率及6个月预后情况。结果观察组患者的并发症发生率为6.67%,明显低于对照组患者的并发症发生率(30.00%),差异有统计学意义(P<0.05);观察组患者在6个月后的预后良好(良好/轻残)率为63.33%,明显优于对照组(33.33%),差异有统计学意义(P<0.05),观察组患者在6个月后的死亡率为10.00%,明显低于对照组(36.67%),差异有统计学意义(P<0.05),两组患者6个月后的预后差(重残/植物生存)率相比无统计学意义(P>0.05)。结论对伴有瞳孔扩大的急性创伤性硬膜下血肿患者实施手术治疗,其并发症相对较多,预后情况较差。Objective To analyze the therapeutic value of acute traumatic subdural hematoma.Methods A total of 60 patients with acute traumatic subdural hematoma treated in Haimen People's Hospital from January 2012 to December 2018 were selected as the research subjects.Thirty patients with unilateral pupillary dilation were selected as the control group,and 30 patients without pupillary dilation were selected as the observation group.Both groups of patients received craniotomy hematoma removal and standard large bone flap decompression.Results The incidence of complications in the observation group was 6.67%,significantly lower than that in the control group(30.00%),with statistically significant difference(P<0.05).The patients in the observation group had a good prognosis(good/light disability)rate of 63.33%after 6 months,which was significantly better than that in the control group(33.33%).the difference was statistically significant(P<0.05).the patients in the observation group had a mortality rate of 10.00%after 6 months,which was significantly lower than that in the control group(36.67%).the difference was statistically significant(P<0.05).the patients in the two groups had a poor prognosis(heavy disability/plant survival)rate after 6 months,which was not statistically significant(P>0.05).Conclusion Surgical treatment for acute traumatic subdural hematoma with pupil dilation has relatively more complications and poorer prognosis.
