摘 要:目的:了解抗肿瘤辅助用药的临床应用合理性。方法:随机选取2017年1月至2018年6月沧州市中心医院(以下简称"我院")肿瘤科接受放化疗治疗的恶性肿瘤患者430例,对其病历进行回顾性调查分析,评价抗肿瘤辅助用药的应用合理性。结果:2017年1-6月、2017年7-12月和2018年1-6月,我院抗肿瘤辅助用药的用药品种数、销售金额、销售金额增长率及销售金额占肿瘤治疗药物总销售金额的比例均呈升高趋势,其中,护肝药、止吐药的销售金额构成比依次降低,扶正祛邪类中药制剂、免疫调节剂的销售金额构成比依次升高,护胃抑酸药、升血细胞药的销售金额构成比均先降低后升高;我院抗肿瘤辅助用药的用药频度(defined daily dose system,DDDs)依次升高,其中,护肝药、护胃抑酸药和升血细胞药的DDDs构成比依次降低,止吐药的DDDs构成比先升高后降低,扶正祛邪类中药制剂、免疫调节剂的DDDs构成比依次升高;我院抗肿瘤辅助用药的DDDs依次升高,其中,口服制剂的DDDs构成比依次降低,注射剂的DDDs构成比依次升高。结论:我院抗肿瘤辅助用药的临床应用频率高,存在不合理用药、不规范用药及无指征用药等现象,在极大程度上导致治疗成本增加。临床必须强化抗肿瘤辅助用药知识的培训与教育,促进临床用药管理水平的提升,规范、合理应用抗肿瘤辅助用药。OBJECTIVE: To investigate the rationality of clinical application of anti-tumor adjuvant drugs. METHODS: Totally 430 patients with malignant tumor undergoing chemoradiotherapy in Cangzhou Central Hospital(hereinafter referred to as "our hospital")from Jan. 2017 to Jun. 2018 were randomly extracted. The medical records of the patients were analyzed retrospectively to evaluate the application rationality of anti-tumor adjuvant drugs. RESULTS: From Jan. to Jun. 2017, Jul. to Dec. 2017 and Jan. to Jun. 2018, the number of drug-using varieties, consumption sum, growth rate of consumption sum of anti-tumor adjuvant drugs, and the ratio of consumption sum of anti-tumor adjuvant drugs to the total consumption sum of anti-tumor drugs in our hospital were in an increasing trend. The proportion of consumption sum of liver-protecting drugs and antiemetic drugs decreased gradually, the proportion of consumption sum of traditional Chinese medicine preparations for strengthening the body and eliminating pathogens and immunomodulators increased gradually, the proportion of consumption sum of stomach-protecting acid-suppressing drugs and blood-raising drugs decreased at first and then increased. The defined daily dose system(DDDs) of anti-tumor drugs increased gradually, and the constituent ratio of DDDs of liver-protecting drugs, stomach-protecting acid-suppressing drugs and blood-raising drugs decreased gradually, the constituent ratio of DDDs of antiemetic drugs increased at first and then decreased, the constituent ratio of DDDs of traditional Chinese medicine preparations for strengthening the body and eliminating pathogens and immunomodulators increased gradually. The DDDs of anti-tumor adjuvant drugs in our hospital gradually increased. Among them, the constituent ratio of DDDs of oral preparations gradually decreased, and the constituent ratio of DDDs of injections increased.
