摘　　要：目的探讨人类免疫缺陷病毒(HIV)/乙型肝炎病毒(HBV)合并感染对高效抗逆转录病毒治疗(HAART)的影响。方法回顾性分析2015年1月1日至2016年5月31日在该院关爱门诊进行HAART治疗的92例患者抗病毒治疗的效果,根据感染HBV情况分为单纯HIV感染组(对照组,63例)和HIV/HBV合并感染组(观察组,29例),收集两组患者一般资料,以及HAART治疗前,HAART治疗后6个月,1、2、3年CD4+T淋巴细胞检测结果,药物不良反应,死亡情况等。结果观察组患者中死亡病例、肝损害病例、出现药物不良反应比例均明显高于观察组。两组患者经HAART后CD4+T淋巴细胞计数均明显升高,但两组患者治疗后6个月,1、2、3年CD4+T淋巴细胞计数比较,差异均无统计学意义(P>0.05);两组患者治疗后1年CD4+T淋巴细胞计数变化幅度显著,2、3年CD4+T淋巴细胞计数变化幅度较低,两组患者CD4+T淋巴细胞计数变化幅度比较,差异均无统计学意义(P>0.05)。结论HIV/HBV合并感染对HAART治疗后免疫细胞的变化无明显影响,但出现肝功能损害、药物不良反应的可能性更大。Objective To investigate the effect of HIV/hepatitis B virus(HBV)co-infection on highly active antiretroviral therapy(HAART).Methods Retrospective analysis of 92 cases of patients with HAART,According to the situation of HBV infection,the patients were divided into two group:63 cases of HIV infection group(control group)and 29 cases of HIV/HBV combined infection group(observation group),collect patients′general information,CD4+T cell counts in peripheral blood were detected at 0 year(before treatment),the six month,the 1 nd year,2 th year and 3 th year of HAART,adverse drug reactions,death,etc.Data of the 2 groups were analyzed by SPSS 23.0 statistical analysis software.Results The proportion of death cases,liver injury cases and adverse drug reactions in the observation group was higher than that in the observation group.Two groups of patients after HAART,CD4+T cell count were significantly higher,but there had no statistical significance on 6 months,1,2,3 years bewteen the two group,the CD4+T lymphocyte count changed significantly in the first year after treatment in both groups significantly,but were low in the second year and the third year,the change of amplitude was no statistical significance between two groups.Conclusion HIV/HBV co-infection has no significant effect on the changes of immune cells after HAART treatment,but liver function damage and adverse drug reactions are more likely.
