摘  要:目的探讨能谱CT在痛风石成像中常见的伪影分类及发生率,并分析减少成像中伪影的方法。方法回顾性分析2018年10月至2019年3月于厦门大学附属中山医院影像科行西门子双源能谱CT扫描的194例患者的诊疗资料,以97例痛风患者为研究组,同等97例非痛风患者为对照组。分析标准参数下两组患者的伪影发生率;调节变化无伪影的痛风者与非痛风者的各参数值,观察其伪影及体积变化;依据变化探讨各种伪影降低的方法,以获得最佳诊断图像。结果对照组患者足部指甲、皮肤伪影发生率分别为90.57%、20.75%,与研究组足部患者的81.13%、13.21%比较差异均无统计学意义(P>0.05);对照组手部患者指甲伪影发生率为18.18%,与研究组手部患者的13.64%比较差异无统计学意义(P>0.05);降低最小HU值和骨距离均能使指甲、皮肤伪影及其体积有效的减少或消除;降低最小HU值时痛风石体积也随之明显降低,而骨距离则不发生明显变化;标准参数下指甲、皮肤伪影的体积为19.634 cm3,降骨距离后指甲、皮肤伪影的体积明显降低,为16.096 cm3;亚毫米、血管、金属伪影通过改变重建内核Q系平滑算法,可以有效减少伪影。结论能谱CT对痛风石的检出有较高价值,可以有效提高痛风石检出率。通过调整Gout参数可以有效减少、消除伪影,有助于临床诊断。Objective To investigate the classification and incidence of common artifacts in energy spectral CT imaging for detecting gout tophi,and analyze methods to reduce artifacts in imaging.Methods A retrospective analysis was performed on the diagnosis and treatment data of 194 patients who underwent CT imaging by Siemens dual-source energy spectral CT scanners at the Department of Imaging,Zhongshan Hospital Affiliated to Xiamen University,from October 2018 to March 2019.The data of 97 patients with gout were used as the study group,and 97 patients with non-gout were used as the control group.The incidence of artifacts was analyzed in the two groups of patients under standard parameters.After adjusting the parameters of artifact-free gout and non-gout patients,the changes were observed in artifact rates and volume.Based on the changes,various methods were explored to reduce artifacts and obtain the best diagnostic image.Results Comparison between the control group and the study group on the artifacts rates of nail(90.57%vs 81.13%)and skin(20.75%vs 13.21%)showed that no significant differences were found between the foot patients in both groups(P>0.05).The incidence of nail artifacts in hand patients was 18.18%in the control group,which was not statistically significant compared with 13.64%in the study group(P>0.05).Reducing the minimum HU value and bone distance can effectively reduce or eliminate nails and skin artifacts and their volume.When the minimum HU value is lowered,the volume of gout tophi is also significantly reduced,while the bone distance does not change significantly.The volume of nail and skin artifacts was 19.634 cm3 under standard parameters,while the volume of nail and skin artifacts was significantly reduced to 16.096 cm3 after reducing the bone distance.Artifacts of sub-millimeter,blood vessels,and metal can be effectively reduced by changing and reconstructing the Q-series kernel smoothing algorithm.Conclusion Energy spectral CT has higher value for the detection of gout tophi and can effectively
