全蝎当归汤联合针刺治疗后天性麻痹性斜视的临床研究

摘　　要：目的观察全蝎当归汤联合针刺治疗后天性麻痹性斜视的临床疗效。方法将60例后天性麻痹性斜视患者按随机数字表法分为2组。对照组30例予西医常规治疗;治疗组30例予全蝎当归汤联合针刺治疗。2组均10 d为1个疗程,治疗2个疗程后统计疗效。比较2组治疗前后主要症状复视、眼斜、头晕、恶心及代偿头位评分变化情况,比较2组治疗前后血流变学指标全血黏度(低切)、全血黏度(中切)、全血黏度(高切)及纤维蛋白原水平变化情况。结果治疗组总有效率93.33%,对照组总有效率50.00%,治疗组疗效优于对照组(P<0.05)。2组治疗后复视、眼斜、头晕、恶心及代偿头位评分与本组治疗前比较均降低(P<0.05),且治疗组治疗后各项评分均低于对照组(P<0.05)。2组治疗后全血黏度(低切)、全血黏度(中切)、全血黏度(高切)及纤维蛋白原水平与本组治疗前比较均降低(P<0.05),且治疗组治疗后各指标水平均低于对照组(P<0.05)。结论全蝎当归汤联合针刺治疗后天性麻痹性斜视临床疗效确切,可明显改善患者临床症状,降低血液黏度及纤维蛋白原水平,促进血液循环,提高临床疗效。Objective To observe the effect of Quanxie Danggui decoction combined with acupuncture in the treatment of acquired paralytic strabismus. Methods 60 patients with acquired paralytic strabismus were randomly divided into two groups according to the random number table method. 30 cases in control group were treated by western medicine. 30 cases in treatment group were treated by Quanxie Danggui decoction combined with acupuncture. Both groups were treated 10 days as a course of treatment, after two courses of treatment, the efficacy was counted. The changes of the scores of diplopia, strabismus, dizziness, nausea and compensatory head position were compared before and after the treatment in the two groups, and the changes of the hemorheology indexes of whole blood viscosity(low cut), whole blood viscosity(middle cut), whole blood viscosity(high cut) and fibrinogen level before and after the treatment in the two groups were compared. Results The total effective rate was 93.33% in treatment group and 50.00% in control group. The clinical curative effect of the treatment group was better than that of the control group(P<0.05). After treatment, the scores of diplopia, strabismus, dizziness, nausea and compensatory head position in the two groups were lower than those in the group before treatment(P<0.05), and the scores in the treatment group were lower than those in the control group(P<0.05). After treatment, the whole blood viscosity(low cut), whole blood viscosity(middle cut), whole blood viscosity(high cut) and fibrinogen levels in the two groups were lower than those in the group before treatment(P<0.05), and the level of each index in the treatment group were lower than those in the control group(P<0.05). Conclusion The clinical effect of Quanxie Danggui decoction combined with acupuncture on acquired paralytic strabismus is definite, which can obviously improve the clinical symptoms, reduce the blood viscosity and fibrinogen level, promote the blood circulation and improve the clinical effect.
