伏立康唑与奥美拉唑联合应用引起药源性肝损伤一例

摘　　要：患者70岁女性,因反复咳嗽五年余,加重2月余入住当地医院治疗,给予吸氧、补钾、抗感染、止咳化痰、解痉平喘等治疗,病情未见缓解,来兰州大学第二医院(以下简称兰大二院)治疗,入院1 d,给予拜复乐抗感染治疗,入院第2 d给予伏立康唑+泰能进行抗感染治疗,给予甲强龙+丙种球蛋白抗炎治疗,给予奥美拉唑预防应激性溃疡,入院第3 d,患者出现肝损伤,谷草转氨酶4014 U/L,谷丙转氨酶1014 U/L,考虑该肝损伤可能与伏立康唑与奥美拉唑联合应用相关,遂立即停用伏立康唑,给予保肝、降酶治疗,入院第6 d,患者肝损好转,谷丙转氨酶482 U/L,谷草转氨酶491 U/L,给予卡泊芬净抗真菌治疗,入院第10 d,患者要求出院,自动出院。A 70-year-old female was admitted to the local hospital because of more than 5 years recurrent cough and was aggra⁃vated for more than 2 months.She was given oxygen inhalation,potassium supplement,anti-infection,antitussive and expectorant thera⁃py,antispasmodic and anti-asthmatic therapy and so on.While the condition was not relieved,she came to our hospital for further treat⁃ment.The 1st day she was admitted to the hospital and was given anti infection therapy with bifurcale,and 2nd day,anti-infection therapy with Voriconazole+Taine and anti-inflammatory therapy with Methylprednisolone+Gamma Globulin were used,Omeprazole was given to prevent the stress ulcer;On 3rd day of admission,the patient presented liver injury with glutamic pyruvic transaminase was 4014 U/L and glutamic pyruvic transaminase 1014 u/L.Considering that the liver injury might be related to the combination use of Voriconazole and Omeprazole,Voriconazole was stopped immediately.She was treated with liver protection and enzyme reduction.On the 6 th day of admission,the liver function was improved,glutamic pyruvic transaminase was 482 U/L and glutamic pyruvic transaminase 491 U/L.On the 10th day,the patient required to be discharged and discharged automatically.
