摘　　要：目的探讨为原因不明不孕性患者在治疗中选择最佳的受精方式。方法对2016年1月-2018年12月72周期行常规IVF治疗的原因不明不孕患者精卵共孵育4 h后剥除卵母细胞周围的卵丘颗粒细胞,观察第二极体排出情况,对第二极体排出率<30%或未见第二极体排出的完全受精失败的IVF周期行补救ICSI为实验组,并与同期行half-ICSI的55个周期原因不明不孕患者(对照组)进行比较。结果实验组和对照组的受精率、≥3PN率、正常受精率、卵裂率、优质胚胎率、可移植胚胎率、临床妊娠率和种植率差异均无统计学意义。结论早期补救ICSI可作为常规IVF完全受精失败或低受精率周期的有效补救措施在辅助生殖中应用。Objective To explore the best way of fertilization in the treatment of patients with unexplained infertility. Methods From January 2016 to December 2018, 72 cycles of conventional IVF were used to treat infertile women with unknown causes. After 4 hours of CO incubation,the cumulus granulosa cells around the oocytes were removed, and the second polar body was observed. ICSI was used to remedy the IVF cycles with the second polar body less than 30% or without complete fertilization failure, and 55 cycles of half ICSI were used at the same time Patients with unknown infertility(control group) were compared. Results There were no significant differences in the fertilization rate, ≥ 3 PN rate, normal fertilization rate, cleavage rate, high-quality embryo rate, transplantable embryo rate, clinical pregnancy rate and implantation rate between the experimental group and the control group. Conclusion ICSI can be used as an effective remedy for failure of IVF or low fertilization rate in the week.
