摘　　要：目的探讨螺旋刀片式股骨近端髓内钉术(PFNA)治疗对股骨粗隆间骨折患者髋关节功能的影响。方法选择2015年4月-2019年3月于医院就诊的股骨粗隆间骨折患者57例,根据不同治疗方式分为PFNA组(n=29)与锁定钢板组(n=28),PFNA组患者采用闭合复位PFNA内固定治疗,锁定钢板组患者采用切开复位钢板内固定治疗,比较两组患者的手术情况及恢复效果。结果 PFNA组患者的手术情况及康复效果明显锁定钢板组患者更优(P <0.05)。PFNA组患者治疗后的血清IL-6、IL-10、TNF-a水平显著低于锁定钢板组(P <0.05)。PFNA组患者治疗后的Harris评分显著高于锁定钢板组(P <0.05)。结论闭合复位PFNA内固定术操作简单方便,安全性高,对股骨粗隆间骨折患者造成的损伤小,患者机体炎症反应轻微,术后髋关节功能恢复快,并发症少,治疗效果良好。Objective To investigate the effect of screw blade proximal femoral intramedullary nailing(PFNA) on the function of hip joint in patients with intertrochanteric fracture of femur. Methods 57 cases ofintertrochanteric fracture of femur treated in our hospital from April 2015 to March 2019 were divided into PFNA group(n=29) and locking plate group(n=28) according to different treatment methods. The patients in PFNA group were treated with closed reduction and internal fixation of PFNA,and the patients in locking plate group were treated with open reduction and internal fixation of steel plate. The operation conditions and recovery effects of the two groups were compared. Results The results of operation and rehabilitation in PFNA group were better than those in locking plate group(P < 0.05). The serum levels of IL-6, IL-10 and TNF-a in PFNA group were significantly lower than those in locking plate group(P < 0.05). The Harris score of PFNA group was significantly higher than that of locking plate group(P < 0.05). Conclusion Closed reduction and internal fixation with PFNA is simple, convenient, safe, with little damage to the patients with intertrochanteric fracture of femur, slight inflammatory reaction, quick recovery of hip function, few complications, and good therapeutic effect. It is worth recommending.
