摘 要:目的探讨一种新型柔性扩张大脑皮层的管状牵开器的临床应用效果。方法选择16例脑深部组织病变开颅手术患者并随机分为实验组和对照组,实验组应用新型管状牵开器,对照组使用金属脑压板。记录2组患者的术中出血量、病灶残余、皮肤切口长度、皮层切口面积、是否留置引流管、是否去骨瓣、GOS评分,并进行统计分析。结果实验组皮层切口面积显著小于对照组,差异有统计学意义(P<0.05);2组出血量,病灶残余、皮肤切口长度、是否留置引流管、是否去骨瓣、GOS评分等指标则无统计学差异。结论该新型颅脑手术用管状牵开器可以有效地减少大脑皮层损伤,有利于保留皮层功能。Objective To evaluate the clinical application of a tubular retractor using flexible dilatation of the cerebral cortex.Methods Clinical data of 16 patients with deep brain tissue lesions who underwent craniotomy were retrospectively analyzed.The patients were randomly divided into the experimental and control groups.A new tubular retractor was used in the experimental group,while a metal brain plate was used in the control group.The following factors were statistically analyzed:intraoperative blood loss,lesion residual,skin incision length,cortical incision area,the GOS score,whether the drainage tube was indwelled,and whether the bone flap was removed.Results In the cortical incision area,there was a statistically significant difference between the two groups(P<0.05)with regard to factors such as amount of bleeding,residual lesions,length of skin incision,whether the drainage tube was indwelled,whether the bone flap was removed,and the GOS score.Apart from the aforementioned factors,there were no significant differences between the experimental and control groups.Conclusion The new type of tubular retractor can be used to effectively reduce cortical damage and preserve cortical function.
