基于早期内镜治疗老年重症急性胆源性胰腺炎的临床效果分析

摘　　要：目的观察早期内镜治疗老年重症急性胆源性胰腺炎的临床效果。方法选择2017年10月至2018年11月本院收治的老年重症急性胆源性胰腺炎患者62例作为本研究对象,入院随机分为对照组和实验组,各31例。对照组患者采取保守治疗,实验组患者则采取早期内镜治疗,对两组患者疗效进行比较。结果两组疗效指标恢复时间对比,实验组AMS、AWBS、血糖和肝肾功能指标均低于对照组,差异有统计学意义(P<0.05);两组症状恢复指标和住院时间对比,实验组体温、疼痛缓解、腹部压痛时间均低于对照组,且住院时间也低于对照组,差异有统计学意义(P<0.05)。两组术后疗效指标对比,实验组治愈率高于对照组,且病死率、复发率均低于对照组,差异有统计学意义(P<0.05)。结论采用早期内镜治疗老年重症急性胆源性胰腺炎,疗效显著,不仅能改善指标,降低疼痛,又能缩短住院时间,有效降低患者经济负担,对患者疾病康复具有重要作用。Objective To observe the clinical effect of early endoscopic treatment of severe acute biliary pancreatitis in the elderly.Methods From October 2017 to November 2018,62 elderly patients with severe acute biliary pancreatitis were randomly divided into control group and experimental group,with 31 cases in each group.The patients in the control group were treated conservatively,while the patients in the experimental group were treated with early endoscopic therapy.The curative effect of the two groups were compared.Results Compared with the recovery time of the two groups,the AMS,AWBS,blood glucose and liver and kidney function indexes in the experimental group were lower than those in the control group.There was significant difference between the two groups(P<0.05).The symptom recovery index and hospitalization time of the experimental groups were lower than that of the control group,and the hospitalization time was lower than that of the control group.There was significant difference between the two groups(P<0.05).The curative rate of the experimental group was higher than that of the control group,and the mortality and recurrence rate were lower than that of the control group(P<0.05).Conclusion Using early endoscopy in the treatment of severe acute biliary pancreatitis in the elderly is very satisfactory.It can not only improve the index,reduce the pain,but also shorten the hospitalization time.In order to effectively reduce the economic burden of patients,for patients with disease rehabilitation has an important role.
