摘 要:目的比较血清降钙素原(PCT)与血液细菌培养在菌血症中的诊断价值。方法选取2019年1~6月我院收治的40例疑似菌血症患者作为研究对象,所有研究者均给予血清PCT检测与血液细菌培养检查。比较两种检测方法的灵敏度、特异性、准确率及不同发热时间阳性率。结果40例患者中,经病理确诊为菌血症患者38例,非菌血症患者2例。血清PCT检测结果诊断菌血症患者36例,阴性4例;血液细菌培养检测结果诊断菌血症的患者28例,阴性12例。两种检测方法的灵敏度和特异性比较,差异无统计学意义(P>0.05);血清PCT检测的准确率高于血液细菌培养检测,差异有统计学意义(P<0.05)。菌血症患者发热后24、48、72 h的血清PCT检测阳性率高于血液细菌培养检测,差异均有统计学意义(P<0.05),且随着时间的延长依次升高。结论相比血液细菌培养,血清PCT检测可以提高菌血症患者的诊断准确率,在临床中具有较高的诊断价值。Objective To compare the diagnostic value of serum procalcitonin(PCT)and blood bacterial culture in bacteremia.Methods A total of 40 patients with suspected bacteremia admitted to our hospital from January to June2019 were selected as the research objects,all the researchers were given serum procalcitonin test and blood bacterial culture test.The sensitivity,specificity,accuracy and positive rate of fever in different time were compared.Results Among 40 patients,38 were diagnosed as bacteremia and 2 as non-bacteremia.36 cases of bacteremia were confirmed by serum PCT,4 cases were negative;28 cases of bacteremia were confirmed by blood bacterial culture,12 cases were negative.There were no significant differences in sensitivity and specificity between the two methods(P>0.05).The accuracy of serum PCT was higher than that of blood bacterial culture,the difference was statistically significant(P<0.05).The positive rate of PCT in 24,48 and 72 hours after fever were higher than those in blood bacterial culture,the differences were statistically significant(P<0.05),and with the increase of time,it increased in turn.Conclusion Compared with blood bacterial culture,the detection of serum PCT can improve the diagnostic accuracy of bacteremia patients,and has a higher diagnostic value in clinical.
