桔贝合剂治疗慢阻肺的临床疗效

摘　　要：目的对比桔贝合剂和左旋氧氟沙星治疗慢性阻塞性肺疾病的临床疗效。方法选取我院呼吸内科于2016年8月至2018年8月收治的慢性阻塞性肺疾病患者80例,随机分为观察组与对照组,每组40例。观察组患者口服桔贝合剂,一次10ml,一日3次,对照组给予500mg左旋氧氟沙星和200ml生理盐水静脉滴注,每天1次,两组连续治疗10天。观察临床疗效,对比治疗前后肺功能变化,记录治疗前后痰液量、咳嗽、气促、肺部啰音等临床症状评分变化及不良反应情况。结果观察组显效12例,有效21例,总有效率为82.5%,无效率为17.5%;对照组显效10例,有效19例,总有效率为72.5%,低于观察组,无效为27.5%高于对照组;肺功能改善情况显示两组治疗后临床效果均优于治疗前,治疗后观察组FEV1、FEV1%pred、FEV1/FVC%均优于对照组,两组对比差异有统计学意义(P<0.05);治疗后观察组痰液量、咳嗽频率、气促、肺部啰音评分显示均优于对照组,两组治疗后痰液量、咳嗽频率、气促、肺部啰音评分均优于治疗前;对照组患者出现2例头痛,4例胃肠道不适,1例皮疹患者,观察组未见。结论桔贝合剂治疗效果优于左旋氧氟沙星,可以有效改善肺功能,提高各临床症状评分,是安全高效的中药制剂,值得临床推广。Objective To compare the clinical efficacy of jubei mixture and levofloxacin in the treatment of chronic obstructive pulmonary disease.Methods 80 cases of patients with chronic obstructive pulmonary disease admitted to the department of respiratory medicine of our hospital from August 2016 to August 2018 were randomly divided into observation group and control group,with 40 cases in each group.In the observation group,patients were given jubei mixture orally,once 10 ml,three times a day,while the control group was given 500mg levofloxacin and 200ml normal saline intravenous drip,once a day.The two groups received continuous treatment for 10 days.Observe the clinical efficacy,compare the changes of pulmonary function before and after treatment,and record the changes and adverse reactions of clinical symptom scores such as sputum volume,cough,shortness of breath and rale of lung before and after treatment.Results The total effective rate was 82.5%,and the effective rate was 17.5%.The total effective rate was 72.5%,lower than the observation group,and 27.5%,higher than the control group.The improvement of lung function showed that the clinical effect of the two groups after treatment was better than that before treatment.After treatment,FEV1,fev1%pred and FEV1/FVC%in the observation group were better than those in the control group,and the difference between the two groups was statistically significant(P<0.05).After treatment,the sputum volume,cough frequency,shortness of breath and lung rale score in the observation group were all better than those in the control group.After treatment,the sputum volume,cough frequency,shortness of breath and lung rale score were all better than those before treatment.In the control group,there were 2 cases of headache,4 cases of gastrointestinal discomfort and 1 case of rash,which were not seen in the observation group.Conclusion The treatment effect of jubei mixture is better than levofloxacin,which can effectively improve lung function and improve clinical symptom score.
