基于微信平台的延续护理在老年高血压患者中的应用研究

摘　　要：目的探讨微信平台在老年高血压患者延续护理中的应用效果。方法收集从我院心内科出院的老年高血压患者120例,经随机数字表法将患者分成试验组和对照组,每组60例,试验组通过微信平台接受出院后的延续护理,对照组通过电话随访接受出院后的延续护理;出院后1个月、3个月、6个月分别观察两组患者的治疗依从性、自我管理效能和血压控制情况,进行对照分析。结果两种方法对老年高血压患者出院后的延续护理均有效果,患者出院后1个月、3个月、6个月的自我管理效能和治疗依从性与出院时比较显著增强(P<0.05);两组患者实施延续护理后,患者的自我管理效能、治疗依从性和血压值,试验组与对照组患者同期比较有显著差异(P<0.05)。结论通过微信平台给老年高血压患者进行延续护理优势明显,可有效提高患者自我管理效能,提高患者高血压的治疗依从性,保持血压在正常水平,值得临床推广使用。Objective To explore the application effect of WeChat platform in the continuous nursing of elderly hypertension patients.Methods 120 elderly hypertension patients discharged from the department of cardiology of our hospital,patients were divided into experimental group and control group by random number table method,with 60 patients in each group.The experimental group received continuous nursing after discharged through WeChat platform,and the control group received continuous nursing after discharged through telephone follow-up.Treatment compliance,self-management efficacy and blood pressure control of patients in the two groups were observed at 1,3 and 6 months after discharged for comparative analysis.Results The two methods were effective in the continuous nursing care of elderly patients with hypertension after discharge,and the self-management efficacy and treatment compliance were significantly enhanced at 1,3 and 6 months after discharge(P<0.05).The self-management efficiency,treatment compliance and blood pressure of patients in the two groups were significantly different from those in the control group(P<0.05).Conclusion WeChat platform for the elderly hypertension patients to continue nursing has obvious advantages,can effectively improve the self-management of patients,improve the treatment compliance of patients with hypertension,keep blood pressure at a normal level,it is worthy of clinical utilization.
