补肾祛瘀汤加减对子宫内膜异位症患者腹腔镜术后孕育功能的影响

摘　　要：目的:观察补肾祛瘀汤加减在子宫内膜异位症(EMS)患者腹腔镜术后治疗中的应用效果及对患者孕育功能的影响。方法:将2013年10月至2015年10月我院行腹腔镜下保守性手术治疗的104例EMS患者采用随机数字表法均分为观察组和对照组各52例并给予亮丙瑞林和戊酸雌二醇维持治疗,观察组在此基础上采用补肾祛瘀汤加减进行治疗,疗程6个月经周期,比较两组中医症状积分、血清免疫性指标、不良反应并随访两组患者3年内妊娠情况。结果:术后6个月时,两组下腹坠痛、痛经、性交痛、腰膝酸软、月经量少、眩晕耳鸣以及舌脉象评分明显降低(P<0.05),且观察组各项症状积分低于对照组,差异有统计学意义(P<0.05);两组血清EMAb阳性率和CA125水平明显降低(P<0.05),且观察组血清EMAb阳性率和CA125水平低于对照组,差异有统计学意义(P<0.05);随访结果显示观察组和对照组复发率分别为7.84%和24.49%,妊娠率分别为52.94%和32.65%,两组差异均有统计学意义(P<0.05);观察组和对照组子宫出血发生率分别为3.85%和15.38%,不良反应发生率分别为9.62%和32.69%,两组差异有统计学意义(P<0.05)。结论:补肾祛瘀汤加减用于EMS患者腹腔镜术后治疗可有效减轻患者症状,降低肿瘤免疫标记物水平,改善患者孕育功能,减少药物治疗不良反应发生。Objective: To study the application effect of Modified Bushen Quyu Decoction in the treatment of patients with endometriosis(EMS) after laparoscopic surgery and its effects on the inoculation function of patients.Methods: 104 patients with EMS who underwent laparoscopic conservative surgery in our hospital from October 2013 to October 2015 were divided into observation group and controlled group according to the random number table method,with 52 cases in each group.And they were given leuprolide and estradiol valerate for maintenance treatment,and the observation group was given Modified Bushen Quyu Decoction on this basis,and they were treated for 6 menstrual cycles.The TCM symptoms scores,serum immunological indexes and adverse reactions were compared between the two groups,and the pregnancy conditions within 3 years were followed up in the two groups.Results: At 6 months after operation,the scores of lower abdominal pain,dysmenorrhea,dyspareunia,soreness and weakness of waist and knees,hypomenorrhea,dizziness and tinnitus and tongue pulse were significantly decreased in the two groups(P < 0.05),and the scores of symptoms in the observation group were lower than those in the controlled group(P < 0.05).The serum EMAb positive rate and CA125 level in the two groups were significantly decreased(P < 0.05),and the serum EMAb positive rate and CA125 level in the observation group were lower than those in the controlled group(P < 0.05).Follow-up results showed that the recurrence rates in the observation group and the controlled group were 7.84% and 24.49% respectively,and the pregnancy rates were 52.94% and 32.65% respectively(P < 0.05).The incidence rates of uterine bleeding in the observation group and the controlled group were 3.85% and 15.38% respectively,and the incidence rates of adverse reactions were 9.62% and 32.69% respectively(P < 0.05).Conclusions: Modified Bushen Quyu Decoction for postoperative treatment of laparoscopic surgery in patients with EMS can effectively alleviate the symptoms,reduce the leve
