摘 要:[目的]系统评价核心肌群训练对脑卒中病人平衡和步态功能的效果。[方法]计算机检索The Cochrane Library、PubMed、EMbase、中国生物医学文献数据库(CBM)、中国学术期刊全文数据库(CNKI)、万方数据库及维普中文期刊数据库中关于核心肌群训练对脑卒中病人平衡功能、步态功能康复效果的随机对照试验。采用RevMan 5.3软件进行数据分析。[结果]共纳入13篇文献。Meta分析结果显示核心肌群训练能够明显改善脑卒中病人Berg平衡量表得分[MD=5.86,95%CI(4.22,7.50),P<0.00001]、步行功能分级(FAC)量表得分[MD=0.67,95%CI(0.39,0.96),P<0.00001]和最大步行速度评分[MD=23.31,95%CI(22.07,24.54),P<0.00001]。[结论]当前证据表明,核心肌群训练在短期内能够显著提升脑卒中病人的平衡功能,一定程度上改善其步行功能,但需进一步在高质量、大样本中验证核心肌群训练应用于脑卒中病人平衡和步行功能的有效性。Objective:To systematically evaluate the effects of core muscles training on balance and gait function of stroke patients.Methods:All randomized controlled trials(RCTs)on the rehabilitation effect of core muscles group training on balance and gait function in patients with stroke were searched in the Cochrane Library and the databases of PubMed,EMbase,CBM,CNKI,WanFang and VIP.The qualities of literatures were evaluated by the Cochrane Handbook(5.1.0).And RevMan 5.3 software was used to take an analysis.Results:A total of 13 randomized controlled trials(RCTs)were included.The results of meta-analysis showed that the core muscles group training could significantly improve the score of the Berg balance[MD=5.86,95%CI(4.22,7.50),P<0.00001],the score of FAC scale[MD=0.67,95%CI(0.39,0.96),P<0.00001]and the score of maximum walking speed[MD=23.31,95%CI(22.07,24.54),P<0.00001]of stroke patients.Conclusion:Current evidence shows that core muscles group training in the short term can not only significantly improve the balance function of stroke patients,but also improve their walking function to a certain extent.It is necessary to further verify the effectiveness of core muscles group training on the balance and walking function of stroke patients in more high quality and large sample randomized controlled trials.
