改良型胸腔闭式引流器在中大量胸腔积液患者中急诊救治的临床评价

摘　　要：目的观察改良型胸腔闭式引流器在急诊应用的临床效果和并发症。方法将20GY型浅静脉留置针通过软胶管与一次性负压引流袋连接自制成胸腔闭式引流器。将108例急诊中大量胸腔积液患者分为观察组(60例,采用改良型胸腔闭式引流器引流胸水)和对照组(48例,采用深静脉留置针接引流袋引流胸水)。记录两组患者治疗前后呼吸频率、心率、血压、动脉血气分析、操作时间及并发症。比较两组患者治疗前及治疗24 h后以上资料的差异。结果在治疗前后呼吸频率、心率、平均动脉压、动脉血气分析改变方面治疗组与对照组无差异(P>0.05)。在操作时间方面观察组明显短于对照组(P<0.05),在并发症方面观察组明显少于对照组(P<0.05)。结论用改良型胸腔闭式引流器引流胸水简单易行,操作时间短,并发症极少,适宜于急诊推广应用。Objective to observe the clinical effect and complications of modified thoracic closed drainage device in emergency treatment.Methods 20G Y type shallow vein indwelling needle was connected with disposable negative pressure drainage bag through soft rubber tube to make a thoracic closed drainage device.108 patients with moderate massive pleural effusion in the emergency department were divided into the observation group(60 cases)and the modified thoracic closed drainage device was used to drain pleural effusion,while 48 patients in the control group were treated with deep vein indwelling needle and drainage bag to drain pleural effusion.Respiratory frequency,heart rate,blood pressure,arterial blood gas analysis,operation time and complications were recorded before and after treatment.The data were compared between the two groups before and after treatment.Results there was no difference between the treatment group and the control group in the changes of respiratory frequency,heart rate,mean.arterial pressure and arterial blood gas analysis before and after treatment(P>0.05).In terms of operation time,the observation group was significantly shorter than the control group(P<0.05),and in terms of complications,the observation group was significantly less than the control group(P<0.05).Conclusion drainage of pleural effusion with modified closed thoracic drainage device is simple and feasible,with short operation time and few complications,which is suitable for emergency application,
