摘  要:目的研究心跳骤停(CA)患者苏复后48h神经元特异性烯醇化酶(NSE)与甲状腺激素水平与神经功能预后的关系。方法共60例患者纳入研究,检测复苏后48h的NSE与甲状腺激素,分析两者的相关性,比较这些指标在预后良好与预后不良之间的差异,及对预后的预测价值。结果预后不良的心跳骤停患者复苏后48h的NSE较高、游离三碘甲状腺原氨酸(fT3)与促甲状腺激素(TSH)较低,差异有统计学意义(P<0.05)。所有心跳骤停患者的N SE与T SH存在显著负相关(r=-0.239,P<0.05)。对预后的预测分析,fT3的曲线下面积(AUC)最高,达0.81;其次为N SE,达0.78。结论复苏后48 h的NSE及fT3水平与心跳骤停患者预后相关,可尝试用于预测心跳骤停患者的功能预后。Objective To study the relationship between neuron specific enolase(NSE)and thyroid hormone levels and the prognosis of neurological function 48 hours after resuscitation in patients with cardiac arrest(CA).Method A total of 60 patients were included in the study.NSE and thyroid hormone were measured 48 hours after resuscitation.The correlation between NSE and thyroid hormone was analyzed.The differences between good prognosis and poor prognosis of these indicators were compared,and the predictive value of these indicators for prognosis was also analyzed.Results There were significant differences in NSE,fT3 and TSH 48 hours after resuscitation in patients with poor prognosis(P<0.05).There was a significant negative correlation between NSE and TSH in all patients with cardiac arrest(r=-0.239,P<0.05).The area under curve(AUC)of fT3 was the highest(0.81),followed by NSE(0.78).Conclusion The NSE and fT3 levels at 48 hours after resuscitation are related to the prognosis of patients with cardiac arrest,which can be used to predict the functional prognosis of patients with cardiac arrest.
