丹参舒心胶囊联合地尔硫卓治疗瘀血型难治性心绞痛临床研究

摘　　要：目的探讨丹参舒心胶囊联合地尔硫卓治疗瘀血型难治性心绞痛的疗效及部分作用机制。方法选取2017年3月-2019年3月就诊的瘀血型难治性心绞痛患者180例,随机分为观察组和对照组,每组90例。对照组除常规治疗外给予地尔硫卓口服,90 mg/次,1次/d,观察组在对照组治疗基础上给予丹参舒心胶囊,2粒/次,3次/d口服,连续治疗4周。观察两组病例治疗前后心绞痛相关评分、心电图及血清细胞色素C(Cyt C)、心肌营养素-1(CT-1)、一氧化氮(NO)、内皮素-1(ET-1)、血管性假血友病因子(vWF)及胰岛素样生长因子-1(IGF-1)含量变化。结果两组病例治疗后临床症状疗效、心电图疗效及心绞痛症状各项积分均明显改善(P<0.05),且观察组治疗后上述症状改善程度优于对照组(P<0.05);与本组治疗前比较,两组病例治疗后血清Cyt C、CT-1、vWF及ET-1含量降低,NO、IGF-1含量升高(P<0.05),且观察组上述指标改善更为明显(P<0.05)。结论丹参舒心胶囊联合地尔硫卓治疗瘀血型难治性心绞痛具有良好的临床疗效,值得临床推广。Objective To investigate the curative effect and mechanism of Danshen Shuxin Capsules combined with diltiazem on refractory angina pectoris in blood stasis pattern.Methods 180 patients admitted in the hospital from March,2017 to March,2019 with refractory angina pectoris in blood stasis pattern were randomly divided into the observation group and the control group,with 90 patients in each group.In addition to routine treatment,patients in the control group was orally given diltiazem,90 mg once a day.The observation group was given Danshen Shuxin Capsules for 2 capsules each time,3 times a day,for 4 weeks on the basis of the control group.Angina pectoris related grading scale,electrocardiogram(ECG),serum cytochrome C(Cyt C),cardiotrophin-1(CT-1),nitric oxide(NO),endothelin-1(ET-1),von Willebrand factor(vWF)and insulin-like growth factor-1(IGF-1)were observed before and after treatment.Results After treatment,the clinical symptoms,ECG and angina symptoms scores in both groups were significantly improved(P<0.05);the improvement in the observation group was better than the control group(P<0.05).After treatment,the serum level of Cyt C,CT-1,vWF and ET-1 decreased and the level of NO and IGF-1 increased in both groups(P<0.05),and the changes in the observation group were more significant(P<0.05).Conclusion Danshen Shuxin Capsules combined with diltiazem has good effect in the treatment of refractory angina pectoris in blood stasis pattern,which is worthy of clinical promotion.
