子宫动脉血流参数及游离β-hCG和雌三醇水平对胎儿生长受限的预测价值

摘　　要：目的:研究子宫动脉血流参数收缩期峰值流速/舒张末期流速(S/D)、搏动指数(PI)、阻力指数(RI)及血清游离β-人绒毛膜促性腺激素(β-hCG)、游离雌三醇(uE3)水平预测胎儿生长受限(FGR)的价值。方法:选取2015年4月至2019年4月湖北民族大学附属民大医院收治的235例行常规产前检查并分娩的孕中期(14~21周)孕妇,按产检结果将孕妇分为对照组(正常妊娠,160例)和观察组(FGR,75例),均于孕14~21周行母血清游离uE3、β-hCG及子宫动脉超声多普勒检测。对比两组血清游离hCG、uE3水平及子宫动脉血流参数S/D、PI、RI变化情况,并分析上述因子预测FGR的价值。结果:观察组血清游离uE3水平显著低于对照组(P<0.05),血清游离β-hCG水平及S/D、PI、RI显著高于对照组(P<0.05)。血清游离uE3、β-hCG及动脉血流动力参数S/D、PI、RI预测FGR的ROC曲线下面积分别为0.767、0.787、0.776、0.738、0.728。结论:血清游离β-hCG、uE3及子宫动脉血流参数S/D、PI、RI与FGR密切相关,可作为预测FGR的可靠指标。Objective:To study the predicted value of systolic peak velocity/end diastolic velocity(S/D),pulsatile index(PI),resistance index(RI),serum freeβ-human chorionic gonadotropin(β-hCG)and free estriol(uE3)for fetal growth restriction(FGR).Methods:235 pregnant women(14-21 weeks)who underwent routine prenatal examination and delivered in our hospital from April 2015 to April 2019 were enrolled,and divided into a control group(normal pregnancy,160 cases)and an observation group(FGR,75 cases).uE3,β-hCG and uterine artery ultrasound Doppler were detected at 14-21 weeks of gestation.The serum freeβ-hCG,uE3 and uterine artery blood flow parameters such as S/D,PI and RI were compared between the two groups,and the value of these factors in predicting FGR was analyzed.Results:The levels of serum free uE3 in the observation group was significantly lower than that in the control group(P<0.05),and the levels of serum freeβ-hCG,S/D,PI and RI were significantly higher(P<0.05).The area under the ROC curve of serum free uE3,β-hCG,S/D,PI and RI were 0.767,0.787,0.776,0.738 and 0.728,respectively.Conclusion:Serum freeβ-hCG,uE3 and uterine artery blood flow parameters S/D,PI and RI are closely related to FGR,which can be used as reliable indicators for FGR prediction.
