摘  要:目的:分析新生儿真菌性败血症的危险因素。方法:选取2012年6月至2018年10月河北省廊坊市人民医院儿科新生儿重症监护室收治的3000例新生儿,分为观察组(发生真菌性败血症)和对照组(未发生真菌性败血症),采用Logistic回归分析法确定新生儿真菌性败血症的危险因素。结果:新生儿真菌性败血症发生率为1.07%(32/3000)。两组性别、分娩方式、住院时间、肠外营养支持时间、应用广谱抗生素、抗生素联合用药、机械通气、合并坏死性小肠结肠炎、肺透明膜病及肺炎比较,差异均无统计学意义(均P>0.05)。观察组胎龄<37周、人工受孕、羊水指数<2 cm、极低出生体重儿、外周静脉中心置管(PICC)留置时间≥14 d、广谱抗生素使用时间≥7 d、机械通气时间≥7 d所占比例均高于对照组(均P<0.05),Logistic回归分析显示,上述因素均为新生儿真菌性败血症的独立危险因素(均P<0.05)。结论:导致新生儿真菌性败血症发生的危险因素较多,临床中应给予针对性干预措施,降低真菌性败血症发生风险。Objective:To analyze the risk factors of neonatal fungal sepsis.Methods:A total of 3,000 neonates in our hospital between June 2012 and October 2018 were selected,and divided into an observation group(neonates with fungal sepsis)and a control group(neonates without fungal sepsis).The risk factors were determined by multivariate logistic regression analysis.Results:The incidence of neonatal fungal sepsis was 1.07%(37/3,000).There were no significant differences in the proportion of sex,mode of delivery,hospital days,parenteral nutrition support time,application of broad-spectrum antibiotics,antibiotics combination,mechanical ventilation,necrotizing enterocolitis,hyaline membrane disease and neonatal pneumonia between two groups(P>0.05).The rates of gestational age<37 weeks,artificial conception,amniotic fluid index<2 cm,very low birth weight infants,PICC retention time≥14 days,broad-spectrum antibiotics use time≥7 days and mechanical ventilation time≥7 days were higher in the observation group than those in the control groups(P<0.05).Logistic regression analysis showed that these factors were independent risk factors for neonatal fungal sepsis(P<0.05).Conclusion:There are many risk factors for neonatal fungal sepsis.Clinical targeted measures should be taken to reduce the risk of fungal septicemia.
