摘  要:目的:探讨基于MR扩散加权成像技术的DWIBS、DTI及PROSET序列对腰脊神经的成像价值。方法:选择2018年5—11月间经MRI常规序列扫描证实的腰椎间盘突出症患者67例,所有患者均使用DWIBS、DTI和PROSET序列进行扫描。DWIBS序列进行最大密度投影(maximum intensity projection,MIP)后处理,DTI序列进行扩散张量纤维示踪成像(diffusion tensor tractography,DTT)后处理,PROSET序列进行多平面重建(multi-plannar reconstruction,MPR)后处理,对比3种序列重建后对腰脊神经的显示情况。采用SPSS 16.0软件进行数据分析。结果:DWIBS、DTI、PROSET序列均能显示腰脊神经;3种序列成像质量的主观评分构成比差异有统计学意义(P<0.05);DWIBS、DTI、PROSET序列评分的平均秩次分别为2.25、1.46、2.29;DWIBS与PROSET序列图像质量评分差异无统计学意义(P>0.05),DWIBS与PROSET序列图像质量评分均优于DTI序列,差异有统计学意义(P<0.05)。结论:MR扩散加权成像技术对腰脊神经的显示有其独特优势,DWIBS和PROSET序列在对腰脊神经的形态学显像上更具优势,DWIBS序列在对腰脊神经全貌的显示上占优,DTI序列对腰脊神经内部结构的定量评估独具价值。3种序列有效弥补了常规MRI序列在腰脊神经成像中的不足,为临床工作提供了有效的影像学依据。Objective To explore the values of DWIBS,DTI and PROSET sequences based on diffusion-weighted MRI for display of lumbar spinal nerves.Methods From May to November 2018,67 patients with lumbar disc herniation confirmed by routine MRI sequence were selected.All the patients were scanned with DWIBS,DTI and PROSET sequences of diffusion-weighted MRI.DWIBS sequence was post processed with maximum intensity projection(MIP),DTI sequence was post processed with diffusion tensor tracing(DTT),and PROSET sequence was post processed with multi-plannar reconstruction(MPR),and the display of lumbar spinal nerves were compared by the three sequences.SPSS 16.0 software was used for data analysis.Results DWIBS,DTI and PROSET sequences could show the lumbar spinal nerve;the subjective scores of the three sequences had statistical significance(P<0.05);the average ranks of DWIBS,DTI and PROSET sequence scores were 2.25,1.46 and 2.29,respectively;there was no statistical difference between DWIBS and PROSET sequence image quality scores(P>0.05),and DWIBS and PROSET sequence image quality scores were higher than DTI sequence,and the differences were statistically significant(P<0.05).Conclusion Diffusion-weighted MRI gains advantages in displaying lumbar spinal nerves.DWIBS and PROSET sequences behave well in morphological imaging of lumbar spinal nerves;DWIBS sequence is superior in displaying the whole pictures of lumbar spinal nerves,and DTI sequence has unique value in quantitative evaluation of the internal structures of lumbar spinal nerves.Three kinds of sequences effectively make up for the shortcomings of conventional MRI sequences in lumbar spinal neuroimaging.
