β2微球蛋白联合尿系列蛋白检测在无症状高尿酸血症肾损伤诊断中的应用及其临床意义

摘　　要：目的探讨血β2微球蛋白联合尿系列蛋白检测在无症状高尿酸血症肾损伤诊断中的价值。方法选取2016年3月至2018年1月于该院接受治疗的无症状高尿酸血症患者共98例,按照随机数字表法分为观察组、对照组,每组49例。对照组采用尿系列蛋白[微量清蛋白(MA)、免疫球蛋白(Ig)、转铁蛋白(TRU)和α1-微球蛋白(α1-MG)]诊断肾损伤,观察组在对照组的基础上联合血β2微球蛋白诊断肾损伤。选择同期入院检查的健康体检者49例为健康组,并接受血β2微球蛋白联合尿系列蛋白检测,比较3组研究对象以上指标的水平,以及对照组与观察组采用不同指标诊断肾损伤的阳性预测值、阴性预测值、特异度、灵敏度、约登指数。结果与健康组比较,观察组、对照组血β2微球蛋白、MA、Ig、TRU、α1-MG水平明显升高,差异有统计学意义(P<0.05);观察组、对照组阴性预测值、阳性预测值比较差异无统计学意义(P>0.05);而观察组特异度、灵敏度、约登指数明显高于对照组,差异有统计学意义(P<0.05)结论联合检测β2微球蛋白及尿系列蛋白对于无症状高尿酸血症肾损伤患者的诊断具有较高的价值,可有效提高检测的灵敏度与阳性预测值,对疾病的早期发现具有重要的临床意义。Objective To explore the application and clinical significance ofβ2 microglobulin combined with urinary series protein in the diagnosis of renal damage caused by asymptomatic hyperuricemia.Methods A total of 98 patients with asymptomatic hyperuricemia who were treated in our hospital from March 2016 to January 2018 were selected and divided into observation group and control group according to randolm number table method,with 49 cases in each group.Patients in control group used series urine protein,including micro albumin(MA),immunoglobulin(Ig),transferrin(TRU)andα1-microglobulin(α1-MG)examination to diagnosis renal impairment.Patients in observation group used the combined detection of series urine protein andβ2 microglobulin to diagnosis renal impairment.And 49 cases of healthy individuals were chosen as healthy group,who accepted the combined detection of series urine protein andβ2 microglobulin as well.The above indexed levels were compared in the three groups,the positive predictive value,negative predictive value,sensitivity,specificity and Youden index were compared beteewn the control group and observation group.Results Compared with the healthy population,the levels ofβ2 microglobulin,urinary series protein MA,Ig,TRU andα1-MG in the blood of patients with hyperuricemia were significantly higher(P<0.05).There were no significant differences on negative predictive value and positive predictive value between the observationgroup and the control group(P>0.05).The specificity,sensitivity and Youden index in the observation group were significantly higher than those in the control group(P<0.05)Conclusion Combined detection of series urine protein andβ2 microglobulin can effectively improve the detection sensitivity and positive detection rate,and has important clinical significance for the early detection of disease.
