膀胱破裂致急性肾功能衰竭6例临床分析

摘　　要：目的探讨腹膜内型膀胱破裂致急性肾功能衰竭的原因,提高对于腹膜内型膀胱破裂的认识,避免误诊误治。方法回顾性分析6例腹膜内型膀胱破裂致急性肾功能衰竭患者的病例资料,分析误诊原因及治疗转归。结果6例膀胱破裂患者均被误诊为急性肾功能衰竭,自发病到确诊的时间为1~7 d,所有患者均急诊行膀胱破裂修补术,腹腔冲洗引流术,至术后1~3 d,5例患者肾功能均恢复正常。1例术后1周因膈下脓肿再次手术清除脓肿后治愈,1例术后出现麻痹性肠梗阻,发展为多脏器功能不全,经支持治疗后痊愈。结论临床上有部分急性肾功能衰竭是因腹膜内型膀胱破裂延误诊断及治疗造成的,应提高对于该病的认识,避免误诊误治。Objective To discuss the causes of acute renal failure caused by intraperitoneal bladder rupture,and improve the understanding of this disease and avoid misdiagnosis and treatment.Methods Clinical data of six patients with intraperitoneal bladder rupture were analysed,retrospectively.Results Six patients were misdiagnosed as acute renal failure.It took one to seven days until forming a correct diagnosis.All patients accepted emergency repair of the bladder rupture and abdominal cavity flusing drainage.The renal function recovered within one to three days after the operations.One patient received another operation because of subphrenic abscess.One patient suffered multiple organ dysfunction syndrome because of paralytic ileus after operation and recovered by supportive treatment.Conclusion Some patients with acute renal failure were caused by intraperitoneal bladder rupture.We should improve the understanding of this disease and avoid misdiagnosis and treatmen.
