肺癌最大标准化摄取值与Ki-67及肿瘤代谢体积的相关性研究

摘　　要：目的:探讨结节型肺癌在^18F-FDG PET/CT显像时最大标准化摄取值(SUVmax)与细胞增殖指数Ki-67和肿瘤代谢体积(MTV)的相关性。方法:回顾性分析2017年1月~2019年3月因肺结节(直经≤3 cm)在本院行^18F-FDG PET/CT显像,随后在本院手术确诊为肺癌的60例患者。收集患者的SUVmax、MTV及Ki-67等相关资料,进行统计学分析。结果:60例患者中男性40例,女性20例,年龄34~78岁,平均年龄61.9±8.7岁,其中腺癌47例,鳞癌7例,肺类癌2例,小细胞癌、大细胞癌、肉瘤样癌、腺鳞癌各1例。SUVmax与Ki-67的总体相关性为r=0.604(P<0.001);SUVmax与MTV的总体相关性为r=0.617(P<0.001)。腺癌中,SUVmax与Ki-67的相关性为r=0.577(P<0.001);SUVmax与MTV的相关性为r=0.565(P<0.001)。结论:在结节型肺癌患者中,病灶的SUVmax与Ki-67、MTV均呈正相关。使用SUVmax评价肿瘤的恶性程度以及预后时,应充分考虑二者的影响。Objective:To explore the correlation between maximum standardized uptake value(SUVmax)and the cell proliferation index Ki-67 or tumor metabolic volume(MTV)in ^18F-FDG PET/CT imaging of nodular lung cancer.Methods:A total of 60 patients who received 18 F-FDG PET/CT imaging in our hospital due to pulmonary nodules(diameter≤3 cm)from January 2017 to March 2019,were analyzed retrospectively,and subsequently diagnosed as lung cancer by surgery.Data of SUVmax,MTV and Ki-67 were collected and analyzed statistically.Results:Among the 60 patients,there were 40 males and 20 females,with age ranged from 34 to 78 years old and an average age at 61.9±8.7 years old.There were 47 cases of adenocarcinoma,7 cases of squamous cell carcinoma,2 cases of pulmonary carcinoid carcinoma,and 1 case of small-cell carcinoma,large-cell carcinoma,sarcomatoid carcinoma and adenosquamous cell carcinoma,respectively.The overall correlation between SUVmax and Ki-67 was r=0.604(P<0.001).The overall correlation between SUVmax and MTV was r=0.617(P<0.001).In adenocarcinoma,the correlation between SUVmax and Ki-67 was r=0.577(P<0.001),and the correlation between SUVmax and MTV was r=0.565(P<0.001),respectively.Conclusion:In patients with nodular lung cancer,SUVmax of the nidus is positively correlated with Ki-67 and MTV.When evaluating the malignancy degree and prognosis of the tumor with SUVmax,the influence of Ki-67 and MTV should be fully considered.
