房颤和高血压引起的左心房增大与血浆脑钠肽之间的关系

摘　　要：目的研究房颤、高血压引起患者左心房增大与血清脑钠肽(brain natriuretic peptide)测量值之间的关系。方法回顾性选取单纯房颤患者131例为房颤组,高血压患者322例为高血压组,每组内再分成左心房增大组和正常组,每位患者均测量其左心房内径和血清脑钠肽水平,分别比较房颤组和高血压组中左心房增大与正常时的血清脑钠肽测量值的差异及其相关性。结果组内比较:高血压、房颤组中的左心房增大组与正常组的BNP测量值比较差异均具有统计学意义(P<0.05),即左心房增大与血清BNP增高有关;组间比较,房颤与高血压无论左心房大小,BNP测量值比较差异均具有统计学意义(P<0.05),即房颤与血清BNP增高有关;左心房大小与血清BNP呈正相关(r=0.208,P<0.05)。结论房颤和左心房增大为引起血清BNP增高的危险因素,且左心房大小与其含量呈正相关。通过测量左心房大小和房颤的诊断对血清BNP增高与否的早期预判、危险分层及原因分析有一定的指导价值。Objective To investigate the relationship between left atrial enlargement induced by atrial fibrillation and hypertension and serum natriuretic peptide. Methods A retrospective selection of pure 131 cases of atrial fibrillation in patients with atrial fibrillation group, 322 cases of high blood pressure, high blood pressure patients group, then each group was divided into the left atrial enlargement group and normal group, the patients were measured the left atrium diameter and the level of serum brain natriuretic peptide, atrial fibrillation and hypertension group were compared when the left atrial enlargement and normal differences of measurement value of serum brain natriuretic peptide and its correlation. Results Intra-group comparison: BNP measurement values of left atrial enlargement group and normal group in hypertension and atrial fibrllation groups were sig-nificantly different(P<0.05), indicating that left atrial enlargement was associated with increased serum BNP. There was significant difference in BNP measurement values between the two groups(P<0.05), regardless of the size of the left atrium between atrial fibrillation and hypertension,indicating that atrial fibrillation was associates with increased serum BNP. The size of the left atrium was positively correlated with serum BNP(r=0.208, P<0.05). Conclusion Atrial fibrillation and left atrial enlargement are risk factors for increased serum BNP, and the size of the left atrium is positively correlated with its content. The measurement of the size of the left atrium and the diagnosis of atrial fibrillation have certain guiding value for the early prediction, risk stratification and cause analysis of serum BNP increase or not.
