摘 要:目的探究皮炎湿疹患者皮损病原菌分布、免疫水平与进展状况。方法以2017年1月-2018年6月本院收治的皮炎湿疹患者作为研究对象,随机选取60例非皮损皮肤皮炎湿疹患者作为非皮损组,选取60例皮损皮肤皮炎湿疹患者作为皮损组,同时选取20例健康体检者作为对照组;分别检测和统计感染病原菌分布、血液Th1/Th2细胞因子水平、T淋巴细胞亚群水平和皮损组患者病损进展状况。结果皮损组细菌总检出率为66.67%,与非皮损组30.00%和对照组25.00%比较差异有统计学意义(P<0.05);马拉色菌检出率皮损组为51.67%,与非皮损组28.33%和对照组25.00%比较差异有统计学意义(P<0.05);金葡菌检出率皮损组为35.00%,非皮损组为31.67%,对照组为0,3组比较差异有统计学意义(P<0.05);表葡菌检出率3组间比较差异无统计学意义(P>0.05)。皮损组IL-2为(83.06±18.03)μg/L,IL-4为(75.51±18.23)μg/L,IL-10为(80.52±17.11)μg/L,IL-12为(90.14±18.26)μg/L,TNF-α为(87.63±20.63)μg/L,IFN-γ为(110.72±14.54)μg/L,均显著高于非皮损组和对照组(P<0.05);非皮损组IL-2、IL-4、IL-10、IL-12、TNF-α、IFN-γ水平均显著高于对照组(P<0.05)。皮损组和非皮损组患者CD3^+、CD4^+T淋巴细胞亚群百分比均显著低于对照组(P<0.05),CD8^+百分比显著高于对照组(P<0.05);非皮损组CD3^+百分比显著高于皮损组(P<0.05),CD8^+百分比显著低于皮损组(P<0.05)。皮损进展患者细菌总密度为(987 263±9 152)cfu/m^2,其中金葡菌密度为(752 389±7 625)cfu/m^2,均显著高于皮损无进展组和对照组(P<0.05);皮损无进展组细菌总密度和金葡菌密度均显著高于对照组(P<0.05)。结论皮炎湿疹皮损患者病情与细菌感染及马拉色菌、金葡菌分布有关,皮损严重程度与相关炎症因子、免疫水平、病原菌密度有密切关系,可为临床诊断和预后评估提供参考。Objectives To investigate the distribution,immune levels,and progression of pathogenic bacteria in patients with dermatitis and eczema.Methods Potential subjects were patients with dermatitis and eczema seen at this Hospital from January 2017 to June 2018.Sixty patients with no dermatitis or eczema were randomly selected to serve as the non-dermatitis group,60 patients with dermatitis and eczema were randomly selected to serve as the dermatitis group,and 20 individuals undergoing a health check-up were selected to serve as the control group.The distribution of infectious pathogens,the level of Th1/Th2 cytokines in blood,the level of T lymphocyte subsets,and the progression of lesions were determined in patients with skin lesions.Results The total rate of detection of bacteria in patients with skin lesions was 66.67%,which differed significantly from 30.00%in patients without skin lesions and 25.00%in the control group(P<0.05).Malassezia was detected at a rate of 51.67%in patients with skin lesions,which differed from 28.33%in patients without skin lesions and 25.00%in the control group(P<0.05).Staphylococcus aureus was detected at a rate of 35.00%in patients with skin lesions,31.67%in patients without skin lesions,and 0%in the control group(P<0.05).There were no significant differences in the rate of detection of Staphylococcus among the three groups(P>0.05).IL-2 was 83.06±18.03μg/L,IL-4 was 75.51±18.23μg/L,IL-10 was 80.52±17.11μg/L,IL-12 was 90.14±18.26μg/L,TNF-a was 87.63±20.63μg/L,and IFN-gamma was 110.72±14.54μg/L,all of which were significantly higher than levels in patients without skin lesions and the control group(P<0.05).The levels of IL-2,IL-4,IL-10,IL-12,TNF-a,and IFN-r in patients without skin lesions were significantly higher than those in the control group(P<0.05).The percentage of CD3^+and CD4^+T lymphocyte subsets in patients with skin lesions and patients without skin lesions was significantly lower than that in the control group(P<0.05),and the percentage of CD8^+cells was significan
