《外科证治全生集》治疗外科疾病浅析

摘　　要：王维德《外科证治全生集》创立了阴阳为主的外科辨证论治法则,重视疮疡阴阳辨证,首倡阳和解凝散寒的治疗原则,治疗上"以消为贵,以托为畏",精于炮制,善用烈药,贡献家传秘方甚多,至今仍是临床常用方剂,推动了中医外科学的发展,但由于当时社会环境及自身思想的局限性,亦有其不足之处:①专论阴阳辨证,否定凭经论治;②重望诊而轻脉诊,忽略了脉诊的重要作用;③反对采用刀针,主张保守治疗;④自述"所载诸方,皆药到病除,切勿增减。"忽略了辨证论治及三因制宜的中医治则。临证应取其精华,去其不足,才能将王维德经验更好地运用到临床中。In Complete Treatise on Patterns and Treatment in External Medicine,Wang Wei-de established the principle of surgical syndrome differentiation and treatment based on yin and yang,paying attention to sore yin-yang syndrome differentiation.He first advocated the therapeutic principle of harmonizing yang and removing cold.His treatment focused on "eliminating and lifting." He is good at processing,and using strong medicinals.He has contributed a lot of recipes handed down in the family which are still commonly-used formulas,have promoted the development of surgery of TCM,but due to the limitations of the social environment and their own thoughts at that time,it also had its shortcomings:① It focuses on the differentiation of yin and yang,and negates the treatment based on meridians.②It pays more attention to inspection than to pulse diagnosis,and ignores the important role of pulse diagnosis.③It opposes the use of knife acupuncture and advocates conservative treatment.④It states that "all the prescriptions contained in it are effective in eliminating the disease.Do not increase or decrease them." However,it ignores the TCM treatment principle of syndrome differentiation and three factors.Therefore,we should take the essence of Wang Wei-de′s clinical experiences and remove their deficiencies so as to better apply his experience to clinical practice.
