壮医药线点灸治疗寒湿凝滞型痛经临床研究

摘　　要：目的:观察壮医药线点灸治疗寒湿凝滞型痛经的临床疗效。方法:将120例痛经患者按照随机数字表法分为药线点灸组、桂枝茯苓胶囊组和针刺组,每组各40例。药线点灸组给予壮医药线点灸,桂枝茯苓胶囊组口服桂枝茯苓胶囊,针刺组给予针刺疗法。比较三组患者的临床疗效及治疗前后疼痛症状积分情况,检测三组患者血液流变学指标及血栓素B2(thromboxane B2,TXB2)水平。结果:桂枝茯苓胶囊组有效率为87.5%,针刺组有效率为92.5%,药线点灸组有效率为97.5%,药线点灸组有效率优于其他两组,差异有统计学意义(P<0.05)。三组患者治疗后痛经症状积分及TXB2水平低于本组治疗前,且药线点灸组治疗后痛经症状积分及TXB2水平低于其他两组,差异具有统计学意义(P<0.05)。药线点灸组及针刺组治疗后低切水平低于桂枝茯苓胶囊组,药线点灸组中切水平低于桂枝茯苓胶囊组,差异均有统计学意义(P<0.05);针刺组与桂枝茯苓胶囊组比较,差异无统计学意义(P>0.05);药线点灸组高切水平低于针刺组,差异具有统计学意义(P<0.05)。桂枝茯苓胶囊组及针刺组治疗后红细胞(red blood cell,RBC)聚集指数、RBC压积及纤维蛋白原水平高于药线点灸组,RBC变形指数低于药线点灸组,差异具有统计学意义(P<0.05);针刺组治疗后、RBC压积低于桂枝茯苓胶囊组,RBC变形指数高于桂枝茯苓胶囊组,差异具有统计学意义(P<0.05)。结论:壮医药线点灸治疗寒湿凝滞型痛经,可降低患者全血黏度及TXB2水平,改善临床症状。Objective:To observe the clinical curative effect of Zhuang medical thread moxibustion on acupoints on dysmenorrhea of cold dampness stagnation type.Methods:A total of 120 dysmenorrhea patients were divided into the group of medical thread moxibustion on acupoints,the Guizhi Fuling Capsules group and the acupuncture group according to the random number table,with 40 cases in each group.The group of medical thread moxibustion on acupoints was treated with Zhuang medical thread moxibustion on acupoints,while the Guizhi Fuling Capsules group was treated with Guizhi Fuling Capsules for oral administration,and the acupuncture group was treated with acupuncture.The clinical efficacies and the scores of pain symptoms before and after the treatment were compared among the three groups,and hemorheology indexes and thromboxane B2(TXB2) levels were measured in three groups.Results:The effective rate was 87.5%,in the Guizhi Fuling Capsules group,92.5% in the acupuncture group,and 97.5% in the group of medical thread moxibustion on acupoints.The effective rate in the group of medical thread moxibustion on acupoints was better than those of the other two groups,and the difference was statistically significant(P<0.05).After the treatment,the scores of dysmenorrhea symptoms in the three groups were lower than those before the treatment,and the scores of dysmenorrhea symptoms and TXB2 in the group of medical thread moxibustion on acupoints were lower than those in the other two groups,and the difference was statistically significant(P<0.05).After the treatment,the whole blood viscosity low cut level of the group of medical thread moxibustion on acupoints and the acupuncture group was lower than that of the Guizhi Fuling Capsules group,and the whole blood viscosity middle cut level of the group of medical thread moxibustion on acupoints was lower than that of Guizhi Fuling Capsule group,and all the differences were statistically significant(P<0.05).There was no significant difference between the acupuncture group and the Guizhi Fu
