艾灸治疗中风后抑郁症的临床研究进展

摘　　要：艾灸治疗中风后抑郁症,可针对中风后抑郁症的病因病机,达到疏肝理气、活血化瘀、健脾益气、温化痰湿、祛除邪气等作用,主要包括单纯艾灸治疗及以艾灸为主的综合治疗,单纯艾灸包括悬灸和隔物灸;以艾灸为主的综合治疗包括艾灸结合针刺治疗、艾灸结合药物治疗、艾灸结合心理治疗、艾灸结合其他治疗等。目前,艾灸治疗中风后抑郁症的临床研究尚处于初始阶段,试验设计尚有许多不足:①未涉及艾灸效应量的问题。今后可对灸量进行比较,探索出最佳灸量,以增强疗效。②中风后抑郁症辨证分型不明确,治疗原则、治疗方法、选穴原则及疗效标准不统一。可结合现有的研究基础,对本病进行统一的辨证分型和疗效标准化,对每个证型进行不同疗法之间的比较,优化治疗方案,为今后临床疗法的选择提供参考依据。③艾灸方法种类繁多,艾灸方法的选择具有不同的作用,故值得进一步深入探索。④实验样本量小,随机对照不足,随访欠缺等,都需要进一步深入研究。Moxibustion treatment of post stroke depression can aim at the etiology and pathogenesis of post stroke depression,and achieve the functions of soothing the liver and regulating qi,promoting blood circulation and removing stasis,strengthening the spleen and benefiting qi,warming phlegm and removing dampness,mainly including simple moxibustion treatment and comprehensive treatment based on moxibustion.Simple moxibustion includes suspended moxibustion and separated moxibustion.The comprehensive treatment based on moxibustion includes moxibustion combined with acupuncture,moxibustion combined with drug treatment,moxibustion combined with psychological treatment,moxibustion combined with other treatments,etc.At present,the clinical research of moxibustion in the treatment of post stroke depression is still in the initial stage,and there are many shortcomings in the experimental design:The problem of moxibustion effect-amount is not involved.In the future,we can compare the amount of moxibustion and explore the best amount of moxibustion to enhance the curative effect.The syndrome differentiation and classification of post stroke depression are not clear,and the treatment principle,treatment method,acupoint selection principle and curative effect standard are not unified.Based on the existing research basis,we can make a unified syndrome differentiation and curative effect standardization,compare different therapies for each syndrome type,optimize the treatment plan,and provide reference for the choice of clinical therapy in the future.There are many kinds of moxibustion methods,and the choice of moxibustion methods has different effects,so it is worth further exploring.The experimental sample size is small,the randomized control is insufficient,the follow-up is not enough,and so on,therefore,all need further in-depth study.
