医院-社区-家庭一体化护理管理模式应用于维持性血液透析患者的效果分析

摘　　要：目的:探讨医院-社区-家庭一体化护理管理模式应用于维持性血液透析患者的效果。方法:将笔者所在医院于2018年1月-2019年1月收治的120例维持性血液透析患者作为研究对象,按照随机数字表法将其分为两组,每组60例。对照组予以传统护理管理模式,研究组予以医院-社区-家庭一体化护理管理模式。比较两组患者收缩压(SBP)、舒张压(DBP)、味觉敏感度、自我管理行为评分。结果:两组干预前SBP、DBP、味觉敏感度、自我管理行为评分比较差异均无统计学意义(P>0.05);干预后两组SBP、DBP水平均较干预前显著降低,味觉敏感度、自我管理行为评分均显著升高,且研究组SBP水平低于对照组,味觉敏感度、自我管理行为评分高于对照组,差异均有统计学意义(P<0.05)。两组干预后DBP比较差异无统计学意义(P>0.05)。结论:医院-社区-家庭一体化护理管理模式在维持性血液透析患者中应用可降低患者舒张压水平,提升味觉敏感度及自我管理行为。Objective:To explore the effect of Hospital-community-family integrated nursing management model on maintenance hemodialysis patients.Method:A total of 120 patients with maintenance hemodialysis admitted in our hospital from January 2018 to January 2019 were taken as the study objects and divided into two groups according to the random number method,with 60 patients in each group.The control group was given traditional nursing management model,and the research group was given Hospital-community-family integrated nursing management model.Systolic blood pressure(SBP),diastolic blood pressure(DBP),taste sensitivity and self-management behavior scores were compared between the two groups.Result:There were no significant differences in SBP,DBP,taste sensitivity and self-management behavior score between the two groups before intervention(P>0.05).After intervention,SBP and DBP level in the two groups were significantly lower than those before intervention,while taste sensitivity and self-management behavior score were significantly higher,and SBP level in the research group was lower than that in the control group,while taste sensitivity and self-management behavior scores were higher than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in DBP between the two groups after intervention(P>0.05).Conclusion:The application of Hospital-community-family integrated nursing management model in maintenance hemodialysis patients can reduce diastolic blood pressure level,improve taste sensitivity and self-management behavior.
