摘 要:目的:探讨蜂蜇伤患者血淀粉酶(AMY)升高的临床意义及其与预后的关系。方法:选择2014年7月-2018年7月笔者所在科室收治的85例蜂蜇伤患者为蜂蜇伤组,选取同期门诊健康体检者80例为对照组,AMY检测采用酶法,比较两组AMY水平;同时对轻、中、重度蜂蜇伤患者AMY水平进行比较;并比较AMY水平与病死率的关系。结果:蜂蜇伤组AMY水平高于对照组,差异有统计学意义(P<0.01);中重度蜇伤组AMY水平高于轻度蜇伤组,差异有统计学意义(P<0.01);蜂蜇伤患者AMY升高49例,占57.6%(49/85),伴胰腺损伤占35.3%(30/85);与AMY<200 U/L患者相比,AMY≥400 U/L患者病死率明显升高,差异有统计学意义(P<0.05)。结论:对蜂蜇伤患者应注意AMY的水平,AMY水平可能是反应患者病情轻重及预测预后的一个生物学指标。Objective:To investigate the clinical significance of elevated amylase(AMY)in patients with bee sting and its relationship with prognosis.Method:From July 2014 to July 2018,85 patients with bee stings were selected as the bee stings group,and 80 healthy outpatients were selected as the control group.AMY activity was detected by enzyme method,and AMY level was compared between the two groups.Meanwhile,the AMY levels of mild to moderate and severe bee stings were compared,and the relationship between AMY levels and mortality was compared.Result:The level of AMY in bee stinging group was significantly higher than that in control group(P<0.01).The level of AMY in moderate and severe stinging group was higher than that in mild stinging group(P<0.01).AMY increased in 49 cases of bee-injured patients,accounting for 57.6%(49/85),35.3%(30/85)with pancreatic injury.The mortality of AMY≥400 U/L was significantly higher than that of the patients with AMY<200 U/L,the difference was statistically significant(P<0.05).Conclusion:The level of AMY should be paid attention to in patients with bee sting.The level of AMY may be a biological index reflecting the severity of the disease and predicting the prognosis.
