剖宫产术后疤痕子宫再次妊娠阴道试产的临床可行性评价

摘　　要：目的探讨对剖宫产术后疤痕子宫再次妊娠阴道试产价值。方法80例疤痕子宫再次妊娠阴道试产孕妇作为观察组,按照1:1选择同期80例在本院接受实施初次妊娠经阴道分娩成功的80例孕妇予以对比(对照组),记录产妇阴道生产的成功率,及各自阴道分娩结果。结果采取阴道试产的80例产妇中,成功经阴道分娩的有60例,成功率达75.00%(60/80),两组分娩时间(8.78±2.38)小时VS(8.03±2.26)小时、分娩出血量(147.39±25.76)mlVS(138.20±22.09)ml、新生儿评分(9.13±0.26)VS(9.20±0.19)、住院时间(5.27±1.18)天VS(5.02±1.09)天均无差异(P>0.05)。结论在明确阴道分娩指征以及严密产程监测基础上进行阴道分娩对母婴无不良影响,疤痕子宫再次妊娠行阴道分娩的临床疗效及安全性高,是一种切实可行的分娩方式,建议推广应用。Purpose.To explore the feasibility of vaginal trial delivery for scar uterus pregnancy after cesarean section.Method.Eighty pregnant women with scar uterus pregnant again were selected as observation group,According to 1:180 pregnant women who received vaginal delivery of the first pregnancy in our hospital during the same period were compared(control group).The success rate of vaginal delivery of pregnant women and their vaginal delivery results were recorded.Result.Of the 80 women who took vaginal delivery,60 had successful vaginal delivery.The success rate was 75.00%(60/80).There was no difference in the two groups of delivery time o(8.78±2.38)hours VS(8.03±2.26)hours.,amount of bleeding during delivery(147.39±25.76)ml VS(138.20±22.09)ml,neonatal score(9.13±0.26)VS(9.20±0.19),hospitalization time(5.27±1.18)days and VS(5.02±1.09)days(P>0.05).Conclusion.On the basis of definite indication of vaginal delivery and strict monitoring of labor process,vaginal delivery has no adverse effect on mother and infant.The clinical efficacy and safety of vaginal delivery for scarred uterus with second pregnancy are high.It is a feasible delivery mode.It is recommended to be popularized and applied.
