摘　　要：推进医疗服务与养老服务相结合是我国积极应对老龄化、实施健康中国战略作出的重大部署。不同于医院办养老机构、养老机构办医院的医养结合模式,北京市羊坊店街道在居家养老工作中逐渐探索出居家养老服务和居家医疗卫生服务相结合的社区居家医养结合服务模式。该模式以老年人需求为导向,以专业医疗团队为支撑,建立多部门协调的联动机制,将居家老人的医疗卫生服务与养老服务的生活照料服务、精神关爱服务、文化教育服务等各类内容融合,具有示范意义。Promoting the integration of the health and elderly care services is an active response to the aging society and the Health China strategy.Different from the service model of combining health care and elderly care in hospitals or facilities,Beijing Yangfangdian street has explored a service model of the integration of the health care and elderly care at home.This model is oriented by the needs of the elderly,supported by professional medical teams,and establishes the linkage mechanism of multi-department coordination.To some extent,it solves the medical and health service needs of the elderly at home,which is of exemplary significance.
