摘  要:目的:研究血管紧张素转换酶2(ACE2)基因rs2285666多态性与2型糖尿病(T2DM)患者合并糖尿病肾病(DKD)的关系方法:病例对照研究选择2017年7月2018年3月中日友好医院收治的T2DM合并DKD[DKD(+)]患者271例(男145例、女126例)和T2DM不伴DKD[DKD(-)]患者272例(男134例、女138例)为研究对象。用高分辨率溶解曲线法进行ACE2基因rs2285666分型统计分析受试者的临床生化指标和ACE2基因不同基因型及等位基W频率结果:临床资料比较,DKD(+)组病程、吸烟比例、收缩压、血肌酐水平显著高于DKD(-)组,eGFR、Cci,显著低于DKD(-)组(均P<0.05),在男性患者中,DKD(+)组G等位基因频率显著高于DKD(-)组(53.4%vs 37.3%,P<0.01)多因素logistic冋归分析显示,G等位基因是男性DKD的独立危险因素(P<0.05)。在女性加性遗传模型中,GG基因型与DKD相关(P<0.05):结论:ACE2基因rs2285666与男性2型糖尿病患者DKD的发病显著相关,携带G等位基因的T2DM男性患者和GG基因型女性患者更易发生DKD.Objective:To explore the relationship between rs2285666 polymorphism of angiotensin converting enzyme 2(ACE2)gene in Han Chinese patients with type 2 diabetic kidney disease(DKD).Methods:From July 2017 to March 2018,a total of 543 type 2 diabetes mellitus(T2DM)participants were selected from China-Japan Friendship Hospital,inc-luding 264 female and 279 male cases.Male cases were divided into two groups:diabetes without DKD[DKD(-)group,n=134]and with DKD[DKD(+)group n=145].Female cases were divided into two groups:diabetes without DKD[DK D(-)group,n=138]and with DKD[DKD(+)group,n=126].The high resolution melting(HRM)method was established in this study to detect the ACE2 genotype.The relevant clinical data and the frequencies of genotype and allele of ACE2 gene rs2285666 polymorphism between two groups were statistically analyzed.Results:Between the patients with DKD and the patients without,DKD(+)group had significantly higher smoking ratio,SEP,Scr,and lower Ccr and eGFR(P<0.05).Frequency of the G allele in males was significantly higher in DKD(+)group than that in DKD(-)group(P<0.01).Multivariate logistic regression analysis found that G allele was an independent risk factor for DKD in males(P<0.05).Rs2285666 was associated with DKD in female additive model(P<0.05).Conclusion:Rs2285666 polymoiphism of ACE2 gene was associated with the incidence of DKD in male T2DM patients.Male T2DM carrier of G allele of the ACE2 gene and GG genotype of female T2DM patients had higher risk of DKD.
