摘  要:目的三碘甲状腺原氨酸(T3)水平升高可以导致甲亢性心脏病,但其机制尚不清楚。文章拟通过研究T3对心肌B细胞激活因子(BAFF)表达的影响,并探讨其在甲亢性心脏病发病中的可能作用。方法选取健康C57BL/6J小鼠60只,随机数字表法分为实验组和对照组,每组30只。实验组腹腔注射T35μg/10g,1次/d,连续注射42 d。放射免疫检定法检测小鼠血清T3和四碘甲状腺原氨酸(T4)浓度;ELISA测定小鼠血清BAFF水平;计算小鼠心脏指数和心肌细胞横径;免疫组化染色法和Western blot检测小鼠心肌BAFF蛋白、心肌肿瘤坏死因子-α(TNF-α)蛋白的表达;Real-Time PCR检测心肌BAFF mRNA的表达;流式细胞术检测心脏B细胞比例的变化。结果与对照组相比,实验组小鼠血清T3浓度、心脏指数、BAFF、心肌细胞横径明显增加(P<0.05),T4浓度降低(P<0.05)。光镜下可见,对照组小鼠心肌细胞结构正常,实验组小鼠心肌细胞明显肥大、结构紊乱。与对照组BAFF蛋白(0.765±0.164)和TNF-α蛋白(0.537±0.089)表达相比,实验组(1.865±0.290、0.737±0.065)明显升高(P<0.05)。T3与心肌细胞BAFF基因表达和外周血BAFF水平的相关性分析表明T3与心肌细胞BAFF基因表达水平呈正相关,相关系数为0.637(P<0.01),T3与外周血BAFF水平呈正相关(r=0.778,P<0.01)。流式细胞术检测:与对照组心肌B细胞的比例[(12.40±1.09)%]相比,实验组[(16.12±0.631)%]增加(P<0.05)。结论高浓度T3可促进心肌细胞BAFF表达升高,B细胞被激活,炎症反应增加,并出现心肌肥大。Objective High levels of triiodothyronine(T3)can lead to hyperthyroid heart disease,but its mechanism is un-clear.This study aims to investigate the effects of T3 on the expression of B-cell activating factor(BAFF)in cardiomyocytes and to ex-plore its possible role in the pathogenesis of hyperthyroid heart disease.Methods Sixty healthy C57 BL/6 J mice were selected and randomly divided into two groups:the experimental group and the control group.The experimental group received intraperitoneal injec-tion of T3 at 5μg/ml,one time/d,for 42 consecutive days.The concentrations of serum T3 and tetraiodothyronine(T4)were detected by radioimmunoassay;ELISA was used to determine BAFF expression in peripheral blood,and the cardiac index and the transverse diameter of myocardial cells in each group were determined.Immunohistochemistry and Western blot were used to detect the expres-sion of BAFF protein in myocardium and of myocardial tumor necro-sis factor-α(TNF-α)protein;the expression of BAFF mRNA in myocardium was detected by Real-Time PCR;flow cytometry(FCM)was used to detect changes in the proportion of B-cells in the Results Compared with the control group,the serum T3 concentration,cardiac index,BAFF and myocardial cell transverse diameter of the experimental group significantly increased(P<0.05),and the T4 concentration decreased(P<0.05).Under the light microscope,the cardiomyocytes of the control group were normal,while those of the experimental group were hypertrophied and disor-dered in structure.Compared with the control group(0.765±0.164),BAFF protein expression significantly increased in the experimen-tal group(1.865±0.290)(P<0.05).Compared with the control group(0.537±0.089),the expression of TNF-αprotein significantly in-creased in the experimental group(0.737±0.065)(P<0.05).Correlation analysis of T3 with BAFF gene expression in cardiomyocytes and BAFF level in peripheral blood showed that T3 was positively correlated with both the former with a correlation coefficient of 0.637(P<0.01)and the latter
