摘 要:目的分析新生儿呼吸道合胞病毒(Respiratory syncytial virus,RSV)肺炎的流行病学及相关危险因素。方法方便收集2016年1月—2018年1月该院新生儿科收治的142例确诊为新生儿肺炎的临床资料,根据RSV抗原是否阳性分为RSV肺炎组71例和非RSV肺炎组71例,采用Logistic多元回归分析RSV肺炎的危险因素。结果新生儿RSV肺炎单因素相关性分析显示早产儿、低出生体重儿、呼吸道感染患者接触史、秋冬季发病、喂养方式(母乳喂养、人工喂养)因素差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,呼吸道感染患者接触史(OR=1.564,95%CI:1.012~1.677,χ^2=1.985,P<0.05)、秋冬季发病(OR=1.338,95%CI:1.102~1.712,χ^2=1.889,P<0.05;OR=1.712,95%CI:1.226~2.098,χ^2=2.221,P<0.01)为RSV肺炎的独立危险因素,母乳喂养的新生儿RSV肺炎风险降低(OR=0.659,95%CI:0.422~0.850,χ^2=1.793,P<0.05)。结论秋冬季为新生儿RSV肺炎的好发季节,有呼吸道感染患者接触史的新生儿RSV肺炎风险大为增加,母乳喂养可降低新生儿RSV肺炎风险。Objective To analyze the epidemiology and relative risk factors of Respiratory Syncytial virus(RSV)pneumonia in neonates.Methods The clinical data of 142 cases diagnosed as neonatal pneumonia admitted to the neonatology department of the hospital from January 2016 to January 2018 were convenient collected.According to whether the RSV antigen was positive,71 cases were RSV pneumonia group and 71 cases were non-RSV pneumonia group.Logistic plural was used.Regression analysis of risk factors for RSV pneumonia.Results Univariate analysis:premature infants,low birth weight infant,contact with patients of respiratory tract infection,autumn and winter infection,the type of feeding(breast feeding and artificial feeding)were considered statistically significant(P<0.05).Multivariate Logistic regression analysis showed that factors increasing the risk of RSV pneumonia were the contact history of patients with respiratory tract infection(OR=1.564,95%CI:1.012-1.677,χ^2=1.985,P<0.05),autumn and winter infection(OR=1.338,95%CI:1.102~1.712,χ^2=1.889,P<0.05;OR=1.712,95%CI:1.226-2.098,χ^2=2.221,P<0.01),and breastfeeding was a protective factor for neonatal RSV pneumonia(OR=0.659,95%CI:0.422-0.850,χ^2=1.793,P<0.05).Conclusion The prevalence of RSV infection was the highest in autumn and winter,and RSV pneumonia occurrence greatly increased in newborns with exposure patients with respiratory tract infection,breastfeeding can reduce the risk of RSV pneumonia in newborns.
