摘  要:目的探析早期肾功能损害诊断中应用尿微量蛋白(U-MTB)与血清胱抑素C(Cys C)联合检验的临床效果。方法方便选取2016年1月—2019年4月期间来该院就诊的62例疑似早期肾功能损害患者为研究对象,根据合并疾病不同分为两组,合并糖尿病者31例,定为观察组A,合并高血压者31例,定为观察组B,同时选取同一时期来该院体检的健康者31名为参照组,对所有对象的U-MTB、Cys C水平进行检测,观察比较各组检测水平及检测准确度、灵敏度、特异度。结果观察组A、观察组B的U-MTB、Cys C水平显著高于参照组,观察组A显著高于观察组B(P<0.05)。在阳性率检测方面,二者联合检测高于单独检测(χ^2=4.010、4.060、4.520、4.630,P<0.05)。62例患者中,确诊为阳性者56例,U-MTB检测确诊为阳性者47例,准确度、灵敏度、特异度分别为72.58%、76.79%、33.33%;Cys C检测确诊为阳性者45例,准确度、灵敏度、特异度分别为64.52%、71.43%、16.66%;二者联合检测确诊为阳性者55例,准确度、灵敏度、特异度分别为91.94%、94.64%、66.67%,二者联合检测结果显著高于单独检测结果(P<0.05)。结论早期肾功能损害诊断中应用U-MTB与Cys C联合检验的准确性更高,可为临床工作提供可靠参考。Objective To explore the clinical effect of urinary microprotein(U-MTB)combined with serum cystatin C(Cys C)in the diagnosis of early renal impairment.Methods 62 patients with suspected early renal impairment who came to the hospital from January 2016 to April 2019 were convenient selected as the research objects.According to the different complications,they were divided into two groups:31 patients with diabetes mellitus,31 patients with hypertension and 31 patients with hypertension,and 31 healthy people who came to the hospital for physical examination in the same period were selected as the reference group.U-MTB and Cys of all the subjects were divided into two groups.C level was detected,and the detection level,accuracy,sensitivity and specificity of each group were observed and compared.Results The levels of U-MTB and Cys C in observation group A and B were significantly higher than those in control group,while those in observation group A were significantly higher than those in observation group B(P<0.05).In the positive rate detection,the combined detection was higher than the single detection(χ^2=4.010,4.060,4.520,4.630,P<0.05).Among 62 patients,56 were positive,47 were positive by U-MTB test,with accuracy,sensitivity and specificity of 72.58%,76.79%and 33.33%,respectively;45 were positive by Cys C test,with accuracy,sensitivity and specificity of 64.52%,71.43%and 16.66%,respectively;55 were positive by combined detection of Cys C and U-MTB,with accuracy,sensitivity and specificity of 91.94%respectively.94.64%and 66.67%respectively.The results of combined test were significantly higher than those of single test(P<0.05).Conclusion The accuracy of U-MTB combined with C ys C test in the diagnosis of early renal impairment is higher,which can provide reliable reference for clinical work.
